About this file

This document is a Federal Supply Schedule Limited-Sources Justification for software operations and maintenance (O&M) support services for the Virgin Islands Department of Health's Immunization Information System (IIS). The contract will be awarded to Optimoz, Inc. under GSA Multiple Award Schedule Information Technology - IT Services with Schedule Identification Numbers 54151HEAL and 54151S. The total estimated contract value is $2,603,604.14, covering a 12-month base period and four 12-month option periods from September 1, 2025, through August 31, 2030.

The services include IIS/system maintenance and ongoing support for the Wisconsin Immunization Registry (WIR)-based software implemented on the Virgin Islands Immunization Information Registry System (VIIRS). Key justifications for sole-source selection include Optimoz's existing expertise with the Virgin Islands' proprietary software, the complexity of migrating to a new system, and the potential disruption to public health operations. The document emphasizes that changing vendors would require a 12-24 month transition period, potentially requiring simultaneous platform maintenance and risking data loss, making Optimoz the most suitable contractor for continued support.

This requirement is for software operations and maintenance (O&M) support services under the General Services Administration (GSA) Multiple Award Schedule (MAS) Information Technology - IT Services with Schedule Identification Number (SIN) 54151HEAL and SIN 54151S. There are no software licenses associated with the anticipated task order contract because the Virgin Island owns the existing Virgin Island Department of Health, Immunization Program (VIDOP) which includes an Immunization Information System (IIS).

The Government does not anticipate that the Base Period O&M support services will differ significantly from those required during Option Periods 1–4. These services include IIS/system maintenance and ongoing support for the WIR-based software implemented on the VIIRS. This support ensures that routine bugs and fixes are addressed so the VIIRS can operate continuously (24/7) in its secure, cloud-based environment. Additional responsibilities include providing upgrades, Template for a Federal Supply Schedule (FSS) Limited-Sources Justification

Completion Instructions: HHS has established a standard template for the preparation of a Limited-Sources Justification (LSJ) pursuant to FAR Subpart 8.4. This template is for use with orders issued under the Federal Supply Schedule (FSS) program where the value exceeds simplified acquisition threshold and competition is restricted: (1) to fewer schedule contractors than required in FAR 8.405-1, 8.405-2, or 8.405-3, or (2) to an item peculiar to one manufacturer. See FAR Subpart 8.4 for additional guidance. This template is not for use with acquisitions other than those under FAR Subpart 8.4.

Tailor all aspects of this template to the individual acquisition. Consult FAR 8.405-6 for guidance. An addendum may be added in case additional space is needed.

When this document is printed, the blue italic instructions are automatically deleted on the printed version.

Virginia O'Garro, Public Health Analyst (Extramural) COR II

2600 Tower Oaks Blvd. Suite 610

These services include IIS/system maintenance and ongoing support for the WIR-based software implemented on the VIIRS. This support ensures that routine bugs and fixes are addressed so the VIIRS can operate continuously (24/7) in its secure, cloud-based environment. Additional responsibilities include providing upgrades, modifications, enhancements to the WIR-based code, and updating the VIIRS coding documentation to reflect these changes. The anticipated task order contract Period of Performance is 1 September 2025 through 31 August 2030. This includes a 12-month Base Period (9/1/2025–8/31/2026) and 4 12- month Option Periods (Option Period 1: 9/1/2026–8/31/2027, Option Period 2: 9/1/2027– 8/31/2028, Option Period 3: 9/1/2028 -8/31/2029 and Option Period 4: 9/1/2029 – 8/31/2030).

The estimated value of the proposed action, including all options, is $

Briefly describe the supplies or services to be provided, including make & model number where appropriate. Indicate whether the item to be procured is peculiar to one manufacturer (e.g., a particular brand name, product, or a feature of a product, peculiar to one manufacturer). If applicable, include quantities of supplies or period of performance for services.

Provide information for any options included in the order. If the action is a modification to an existing order, distinguish clearly between the work covered by the basic order and the work to be obtained by the proposed modification.

4. Authority:

ORDER AGAINST FSS: Authority of the Multiple Award Schedule Program, Title III of the Federal Property and Administrative Services Act of 1949 (41 U.S.C. 251, et seq.); Title 40 U.S.C. 501, Services for Executive Agencies; and Section 803 of the National Defense Authorization Act of 2002 (PL 107-107) and implemented by FAR 8.405-6. Mark the applicable exception and for each discuss how the cited authority applies in accordance with FAR 8.405-6(c)(2)(iv).

An urgent and compelling need exists, and following the procedures would result in unacceptable delays (FAR 8.405-6(a)(1)(i)(A));

Only one source is capable of providing the supplies or services required at the level or quality required because the supplies or services are unique or highly specialized (FAR 8.405-6(a)(1)(i)(B));

Optimoz, Inc. is the current provider of operations and maintenance (O&M) support services for the existing Virgin Islands Department of Health, Immunization Program (VIDOHIP), customized version of the Wisconsin Immunization Registry (WIR) web-based software on the Virgin Islands’ Immunization Information System (VIIRS). The Jurisdictions have made significant financial commitments to their IIS development, and it would be logistically and financially prohibitive for a Jurisdiction to change IIS platforms and/or O&M support services Vendors. Changing IIS platforms would require 12–24 months for the transition, not including time needed for customizing a COTS platform for the individual Jurisdiction’s needs. During the transition period, the Virgin Islands would have to financially support two IIS platforms running simultaneously to ensure no vaccination and/or other public health data is irrevocably lost and/or otherwise damaged. Currently, the Virgin Islands uses Federal funds for all IIS-related support. The Government does not anticipate that the Jurisdiction will have its own funding source(s) to support its IIS beyond this requirement.

In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order provided that the original order was placed in accordance with the applicable Federal Supply Schedule ordering procedures. The original order or BPA must not have been previously issued under sole-source or limited sources procedures (FAR 8.405-6(a)(1)(i)(C)); or

An item peculiar to one manufacturer can be a particular brand name, product, or a feature of a product, peculiar to one manufacturer). A brand name item, whether available on one or more schedule contracts, is an item peculiar to one manufacturer. (FAR 8.405-6(b)).

When competition is limited to items peculiar to one manufacturer, this justification must explain why the particular brand name, product, or feature is essential to the Government's requirements, and that market research indicates other companies' similar products, or products lacking the particular feature, do not meet, or cannot be modified to meet, the agency's needs in accordance with FAR 11.105.

modifications, enhancements to the WIR-based code, and updating the VIIRS coding documentation to reflect these changes. Technical expertise must be provided to maintain uninterrupted operations, deliver training and user support, and ensure an effective transition of work under this Task Order to a follow-on contract at the end of the performance period.

2,603,604.14 (12-month Base Period and 4 12-month

Market research identified a limited number of IIS system Vendors. However, there are no Vendors with the technical capability to provide operations and maintenance (O&M) support services for the Virgin Island’s Department of Health (VIDOHOP) proprietary, jurisdictional-developed software, which is fully integrated in VIDOHOP’s complex public health IT system, and VIIRS. Optimoz, Inc. is a contract holder under the General Service Administration's (GSA) Multiple Award Schedule (MAS) Information Technology - IT Services, with Schedule Identification Numbers (SIN) 54151HEAL and SIN 54151S. The price list for Optimoz Inc, supports the full scope of this requirement. IIS platform services are commercially available and varying degrees of platform support services are offered. None of the commercial-off-the-shelf (COTS) IIS platform services or IIS platforms support VIDOP's complex ,public health IT system and VIIRS which the Virgin Islands uses to provide comprehensive public health support and to integrate with thousands of partner organizations.

Optimoz, Inc. is a small business. There are no other known small businesses that have the technical capability to sucessfully meet the Government's requirement for O&M support services for VIIRS, which is integrated into VIDOH’s complex public health IT system.

Continual IIS needs are driven by the Direct Assistance relationship with the Virgin Islands. The Direct Assistance relationship is continually evaluated, should the Government's support of Virgin Islands end, the Virgin Islands will directly procure follow-on services. Additionally, present technology restricts the ability to disconnect from one system and connect to another system without significant impact to the people of the Virgin Islands. The Government continues to assess the industry and should technological advancements be made to mitigate the impact, the competitive environment will be reassessed.

Moving to one of the other Immunization Information System (IIS) platform options from the Virgin Island’s Department of Health (VIDOHOP) proprietary, jurisdictional-developed software, which is fully integrated in VIDOHOP’s complex public health IT system, and VIIRS to another system such as STC, Envision, or Gainwell carry costs such as, but not limited to, IIS data migration across platforms, 12-24 month installation period requiring concurring IIS platform expense (e.g., paying for two IIS platforms simultaneously), participating organizations and reporting entity on-boarding testing, training the Virgin Island's staff, training participating organizations, to use the new IIS platform in preparation for the IIS migration.

5. Market Research:

Describe the extent of the market research conducted among the schedule holders and the results, or a statement of the reason that market research was not conducted. Brand name specifications shall not be used unless the particular brand name, product, or feature is essential to the Government’s requirements, and market research indicates other companies’ similar products, or products lacking the particular feature, do not meet, or cannot be modified to meet, the agency’s need.

6. Actions to Increase Competition:

Describe the actions, if any, the agency may take to remove or overcome any barriers that led to the restricted consideration prior to any subsequent acquisition for the supplies or services is made. If no such actions have been taken, explain why.
