About this file

This is an Acceptable Quality Level (AQL) inspection form and performance standards document for janitorial services at a Bureau of Land Management facility. The AQL establishes that the contractor must maintain 15 or fewer deficiencies during any 30-day calendar month with a maximum of 3 deficiencies per individual inspection. Failure to perform any specific task outlined in the inspection standards will result in non-compliance with AQL requirements. The Contracting Officer's Representative (COR) or designated project inspector will conduct random inspections of all services performed.

Payment is structured on a deficiency-based scale: full payment is made when deficiencies number 5 or fewer per month; 90% payment when deficiencies range from 6-10; 80% payment when deficiencies range from 11-15; and pro-rated payment based on days meeting AQL standards when deficiencies exceed 15 or when more than 3 deficiencies are documented per inspection. The document specifies detailed inspection standards covering floors (vacuuming, sweeping, mopping, scrubbing), restrooms (fixtures, walls, floors, disinfection), restroom supplies, waste and recycling containers, drinking fountains, glass doors, and windows. Repeated failure to meet AQL standards may result in default termination. The contractor must also ensure annual tile floor cleaning, stripping, and resealing in the Interpretive Center on a designated date.

Exhibit A

Acceptable Quality Level (AQL) (Per E.2.0) The AQL is 15 or fewer deficiencies during a 30 day calendar month with no more than 3 deficiencies per inspection. Failure to perform any specific task in Section C noted during an inspection will result in not meeting AQL. Contractor may be called back by government representative to reperform deficient services. Repeated failure to meet the AQL will be grounds for default termination. Inspection of services performed will be performed on a random basis by the COR or designated project inspector.

PAYMENT (Per E.3.0) Payment will be made at the unit price as shown in Section B in accordance to the following terms: Where the number of deficiencies found during inspections is no more than 5 for all inspections during a calendar month, full payment will be made. Where the number of deficiencies is from 6 to no more than 10, 90% payment will be made. Where the number of deficiencies is from 11 to no more than 15, 80% payment will be made. Where the number of deficiencies is greater than 15, or greater than 3 deficiencies per inspection are documented not meeting AQL, a pro-rated payment will be made after input from government and contractor utilizing the Contract Corrective Action Plan document (Attachment D) based on the days calculated that met AQL at the daily rate.

Where the number of deficiencies exceeds the AQL of 15, the Contractor will be instructed to take necessary action to ensure that future performance conforms to contract requirements. If the Contractor fails to perform to contract requirements the Government may (1) by contract or otherwise, perform the services and charge to the Contractor any cost incurred that is directly related to the performance of such service, (2) issue payment based on the pro-rated calculation for work that met AQL, or (3) terminate the contract for default.
