STATEMENT OF WORK

MOLD REMEDIATION - HORTON AGENCY

1.0 INTRODUCTION

The Bureau of Indian Affairs (BIA) requires the following work be performed at the Horton Agency, 908 1st Avenue East., Horton, KS 66439. See the attachment for a floor plan/schematic of the facility. The contract issued from this solicitation shall be for commercial services and shall be a firm fixed price.

2.0 SCOPE

Description of Requirement: BIA has a mold assessment, cleaning, and reporting requirement for interior building spaces at the Horton Agency, a federally owned facility.

The non-construction services work consists of including all equipment, materials, and labor to clean surface mold observed throughout the interior of the Horton Agency specific to Basement Storage Room and Server Room (approximately 400 sq. ft./”the covered area.”). The work includes, but is not limited to, cleaning and remediation of mold from interior surfaces and materials of observable mold or mold spore-related contaminants, clearance sampling to demonstrate cleaning effectiveness, and final written report. The report shall clearly identify the locations cleaned/remediate and any further work recommendations to prevent future mold growth.

3.0 REFERENCES

All work shall be managed/overseen by a Certified Industrial Hygienist (CIH) and performed by qualified personnel. The work should follow:

· US EPA publication “Mold Remediation in Schools and Commercial Buildings EPA 402-K-01-001, September 2008”.

· ANSI/IICRC S520 Standard for Professional Mold Remediation.

4.0 REQUIREMENTS

The Contractor shall propose a price for the below work via the schedule of items on page 2 and include a proposed project schedule to include the following project phases:

1. Date for completion of mold cleaning, and

2. Date for completion of clearance sampling for cleaning effectiveness and final report.

All dates shall be stated in number of days after contract award.

Cleaning/Remediation Planning and Execution Develop a cleaning/remediation plan and provide to BIA for review and approval. Perform cleaning of identified mold areas identified in the plan and restore interior surfaces to original condition (as applicable). Cleaning work may include demolishing or removing materials (such as carpet, drywall, ceiling tiles, etc.) that cannot effectively be cleaned using surface cleaners. Contractor shall include reinstallation of materials (e.g., new drywall or ceiling tiles) in the cleaning plan and price it appropriately. BIA is able to approve or amend the plan depending on the site’s worker capacity to reinstall materials or equipment themselves. In general, BIA expects the interior to be restored as close as possible to original condition.

Contractor is responsible for securing any additional sampling, such as lead- or asbestos-containing material, that may be required for worker and occupant safety if the materials will be damaged/disturbed as part of the work. If these hazardous materials are present, contractor shall present a revised cleaning/remediation plan. The revised plan may include third party contracting to have materials tested or abated. BIA will review the revised plan and provide any existing material construction or sampling records, if accessible and as needed.

The Contractor shall provide a disposal container or equipment needed to support the removal of all demolished material, waste, and debris. The site shall identify the staging location for containers and equipment needed during remediation. The contractor shall keep the working area in a clean and proper condition. All rubbish and waste resulting from the execution of the work shall be removed at the end of each day or as directed by the site manager.

Clearance Sampling If pre-cleanup sampling occurred, secure, through a third party, post-cleanup clearance sampling to demonstrate effectiveness of cleaning. Per note above in Mold Sampling Plan section, there are no federally recognized standards or thresholds for airborne concentrations of mold. Therefore, contractor shall include in their proposal the mechanism for demonstrating effectiveness of cleaning. This may include resampling to mimic original sampling methods and show, at a minimum, a decrease the concentrations/types of mold. Note that any airborne sampling shall also compare values to an outdoor sample to reflect background mold concentrations/types in the local natural environment.

Regardless of sampling, no visible mold, mold-damaged materials, or moldy odors should be present after cleanup.

Final Report Final report should summarize and evaluate cleaning, actions taken, and contain (as applicable):

· Non-scope best management practices, building repairs, or HVAC equipment Operation & Maintenance recommendations that will reduce or prevent future mold recurrence.

· The objective is to obtain a Certified Industrial Hygiene-level report with conclusions and written certification that facility meets applicable indoor environmental quality standards for safe human occupancy.

Deliverable shall be a written report in pdf format and document cleaning/remediation actions performed, and clearance sampling results. Report shall also outline operational recommendations for building staff to follow (i.e., Operation & Maintenance plan with periodic cleanings/annual assessments) to prevent future mold recurrence. All data, findings and recommendations shall be correlated with a specific system and location and represented on a plan view drawing of the facility.

NOTES:

Contractor shall follow all federal, state and local occupational safety and health regulations, to include engineering controls, personal protective equipment, safe work practices, etc.

Contractor shall follow all federal, state and local environmental regulations regarding disposal or storage of materials. Mold-contaminated waste is not typically classified as hazardous, but contractor shall consult environmental regulations and confirm for generated waste.

If laboratory analysis is warranted, transportation of samples to laboratory shall be conducted under chain-of-custody documentation for analysis and reporting of results.

Distribution of the results of this sampling, testing, and reporting for this facility will be restricted to BIA personnel.
