STATEMENT OF WORK

Mariano Lake Community School
Replace Roofing Project

1.0 Location: Mariano Lake Community School, Navajo Route 49, Crownpoint NM.

2.0 Background: New Mexico Navajo Central Agency Branch of Facilities Management is requesting for roofing membrane replacement on the flat surfaces of Building # 413, 414, and 1399. Contractor shall also access Building 1400 for leakage. Temporary repairs have been done on these roofs and are now requiring new roofs to be installed. The existing membrane is a TPO system on buildings 413 and 1400. Building #414 which is over the boiler room has a modified bitumen torch-down roofing system. Building # 1400 has a panel metal roofing system with numerous leaks throughout.

3.0 Scope: Contractor shall furnish all labor, materials, personnel training/certifications, insurance and equipment to perform the specified services and shall be responsible for the timely completion, quality, technical accuracy and restoration of the area. Contractor shall check in at the front office located in Building # 414 prior to commencement of word every day. The School POC for check in is Thurman Lewis, School Maintenance or Barbara Hanson, Facility Manager/COR. Contractor shall be accompanied by the School POC’s while on school premises.

(Building #1400 – Library) Contractor shall assess the roofing system for leaks on all surfaces of the roof and along the HVAC units. Contractor shall recommend or install seal/gaskets around the HVAC units as they have been identified as leaking.

(Building 414 – Boiler Room) Contractor shall assess the existing modified bitumen torch-down roof system. Contractor shall remove the existing roof and materials around pipes, curbs and walls. Contractor shall clean and prepare existing perimeter of roof edge and install Thermoplastic Polyolefin (TPO) roof system. Contractor shall check existing expansion joint and decking and make any necessary repairs. Contractor shall ensure proper slope for drainage and ensure no ponding will occur on roof. Contractor shall install slope panels to direct water to the scuppers. Contractor shall install a new air barrier prior to installing the R-30 minimum of polyisocyanurate board insulation or equivalent. Contractor shall install a new 60 mil TPO sheet and assemble with mechanical fastener per manufacturers specifications. Contractor shall ensure the TPO membrane is taken over the metal drip edges and terminate at the face of the metal drip edge. Contractor shall install new flashing on vent pipes, curbs, walls and existing equipment where necessary. Contractor shall install pipe curbs, drip and coping caps and fasteners on the TPO roof system.

(Building 413 Dormitory) Contractor shall accompany School COR and assess the flat area of roof on Building #413 (Dormitory). Contractor shall assess the existing TPO membrane for damages, ponding areas and all the exhaust fans and vent pipes. Contractor shall remove the existing TPO membrane and insulation, Contractor shall clean and prepare existing perimeter roof edge to accept new TPO roof system. Contractor shall check existing expansion joint, decking and make any necessary repairs. Contractor shall prepare proper slope for drainage and ensure no ponding on roof. Contractor shall install slope panels to direct water to the scuppers. Contractor shall install new air barrier prior to installing R-30 minimum of polyisocyanurate board insulation or equivalent. Contractor shall install new minimum 60 mil TPO sheets and assemble with mechanical fastener per manufacturers specifications. Contractor shall ensure TPO membrane is over the metal drip edges and terminated at the face of metal drip edges. Contractor shall install new flashing on vent pipes, curbs, walls and existing equipment. Contractor shall install new pipes, curbs, drip and coping caps and fasteners on the roof system.

(Building #1399 – Gymnasium) Contractor shall perform walk through with COR to assess the area. Contractor shall assess the existing TPO membrane for damages, ponding areas to include around the HVAC and ancillary components. Contractor shall remove the existing TPO membrane and insulation, Contractor shall cut and remove existing TPO from pipes, curbs, walls and HVAC equipment. Contractor shall clean and prepare existing perimeter roof edge and check existing expansion joint decking and repairing as necessary. Contractor shall install proper slope for drainage and ensure no ponding is on the roof. Contractor shall install slope panels to direct water to the scuppers. Contractor shall install new air barrier prior to installing R-30 minimum of polyisocyanurate board insulation or equivalent. Contractor shall install new minimum 60 mil TPO sheets and assemble with mechanical fastener per manufacturers specifications. Contractor shall ensure TPO membrane is over the metal drip edges and terminated at the face of metal drip edges. Contractor shall install new flashing on vent pipes, curbs, walls and existing equipment. Contractor shall install new pipes, curbs, drip and coping caps and fasteners on the roof system.

4.0 Work Schedule:

Work shall be performed Monday-Friday between the hours of 8:00am and 4:30pm. Period of performance shall be 60 days from the date of Notice to Proceed.

Contractor shall notify the COR to schedule site visits and to schedule work. Contractor shall make arrangements prior to arriving on-site.

Contractor shall provide work schedule prior to commencement of work and progress schedule once the project has started to the COR.

Contractor shall coordinate with the COR to install temporary safety fencing around work area.

5.0 Project Completion: Contractor shall remove all debris and prepare for off site disposal. Contractor shall coordinate a final walk-through inspection with the COR and provide all warranty information.

6.0 Conclusion: Any Changes to the project scope or other provisions in this Statement of Work must be authorized by the Contracting Officer in writing. Questions and changes can be directed to the COR; however no changes are authorized without written approval from the Contracting Officer.
