STATEMENT OF WORK
SECTION C:
DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK
C.1 SCOPE OF WORK
The Contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and incidental services necessary to perform hearing conservation program training, pure-tone hearing tests, PPE/hearing protection training and review by a certified Audiologist for threshold changes and records retention for approximately 350 employees as described in this Statement of Work (SOW).
These services are to be performed at the following locations:
· Wyoming State Office (approximately 20 employees) 5353 Yellowstone Rd, Cheyenne, WY 82009
· Pinedale Field Office (approximately 24 employees) 1625 Pine St, Pinedale, WY 82941
· Kemmerer Field Office (approximately 19 employees) 430 North Highway 189 Kemmerer, WY 83101-9711
· High Desert District/Rock Springs Field Office (approximately 59 employees) 280 Highway 191 North, Rock Springs, WY 82901-3447
· Rawlins Field Office (approximately 47 employees) 1300 North Third Rawlins, WY 82301-2407

This Contract is a Base plus 4 years optional Contract.
C.2 DEFINITIONS
C.2.1 General Definitions
ACCEPTABLE QUALITY LEVEL (AQL). Maximum deviation from the performance standard.
PERFORMANCE BASED ACQUISITION. A contracting method intended to ensure that required performance quality levels are achieved and that the Contractor’s total payment is related to the degree services meet contract performance standards.
PERFROMANCE REQUIREMENTS SUMMARY (PRS). An abridged version (summary) of the Performance Work Statement showing contract requirements, tasks/subtasks, standards and acceptable quality levels. The PRS may also contain an abridged version of Quality Assurance Surveillance Plan showing the Government’s method of surveillance, incentives, and fee reductions for not meeting standards.
PERFORMANCE STANDARD. Standard of performance required by the Government.
STATEMENT WORK (SOW). Specific requirements the Contractor must meet in performance of the contract. The SOW contains required services (tasks/critical subtasks), performance standards, acceptable quality levels, and incentives/disincentives.
QUALITY ASSURANCE. Actions taken by the Government to assure services meet contract requirements.
QUALITY ASSURANCE SURVEILLANCE PLAN (QASP) OR QUALITY ASSURANCE PLAN (QAP). A written document showing Government surveillance method and scheduling to be used to assess the Contractor’s performance against the performance standard.
QUALITY CONTROL. Those actions taken by a Contractor to control the performance of services to ensure services meet the requirements of the SOW.
C.2.2 Technical Specifics
None
C.3 GOVERNMENT-FURNISHED PROPERTY
Electrical, water
C.3.1
Contractor will have available, electrical and water usage at each office. If a 3-phase electrical is needed, Contractor shall advise the CO 5 working days in advance.
C.4 CONTRACTOR FURNISHED ITEMS
Contractor shall provide a schedule at a prework meeting, set up by the Contractor within 10 days of the notice to proceed. The schedule shall be kept up to date and any changes shall be reported to the CO immediately.
Contractor shall provide all equipment needed to perform the work.
C.4.1 Quality Control Plan
The Contractor shall establish and maintain a Quality Control Program to ensure the requirements of the contract are achieved. One copy of the Contractor’s Quality Control Plan shall:
Be provided with the Offeror’s initial proposal (see Section L & M)
C.5 TASKS/CRITICAL SUBSTASKS AND STANDARDS
C.5.1 Task 1: Hearing Conservation Program training
Standard: Train 100% of the employees on hearing conservation as it relates to OSHA, workplace, PPE, and personal life. Contractor shall review all training material with the COR before going to the sites. Training material shall be able to be understood by employees that are new to hearing conservation.
AQL: Random sample of 5% of employees will be given a 2-question test to see if they understood the material. Contractor and COR will come up with the questions during the prework meeting.
C.5.2 Task 2: Pure-tone hearing tests

C.5.3 Task 3: PPE/hearing protection training
| Standard: Training meets OSHA requirements for yearly hearing protection PPE training. |
| AQL: Safety personnel throughout the State will report back to the COR on the training. |
C.5.4 Task 4: Review of individual hearing tests for threshold changes. Shall be conducted by Audiologist.
| Standard:100% of employees with changes outside of the threshold, shall have a personal, private consultation from an Audiologist per OSHA requirements. |
| AQL: Report from the Audiologist to the State Safety Officer on threshold changes report as per OSHA requirements. |
C.5.5 Task 5: Record retention
| Standard: Contractor retains all records for a minimum of 30 years |
| AQL: State Safety Officer will review 5% random sample of records every year. |
SECTION L
INSTRUCTIONS, CONDITIONS, AND NOTICES TO OFFERORS
L.1 TECHNICAL PROPOSAL CONTENTS
QUALIFICATIONS AND EXPERIENCE OF KEY PERSONNEL: Offerors shall submit a one-page resume of all key personnel proposed for this contract. For this contract, the following positions have been determined as key:
Must be performed by a
· Licensed or certified audiologist, otolaryngologist, or other physician, or;
· A technician who is certified by CAOHC Technicians:
· Must be responsible to an audiologist, otolaryngologist, or physician
· May not perform the duties that specifically require and audiologist, otolaryngologist, or physician, such as reviewing problem audiograms, determining if further evaluation is needed and determining work-relatedness of an STS
PAST PERFORMANCE: Offerors shall submit a list of clients for past and present similar projects covering the past three (3) years. Provide: contact name, address, phone number, fax number, e-mail address. The Government will survey these clients to obtain past performance information (quality, timeliness, cost control, business relationships) related to Offeror’s current and past work.
QUALITY CONTROL PLAN: Offeror’s shall submit their Quality Control Plan showing how they intend to meet Government performance standards for each task and subtask associated with this contract.
