1.
STATEMENT OF-WORK
FOR
REHAB WET LAB PHASE TWO (CONSTRUCTION)
At The
WARM SPRINGS NATIONAL FISH HATCHERY
5308 Spring Street Warm Springs, Georgia 31830
6/20/2023
1. GENERAL
a. Rehabilitate Wet Lab at the Warm Springs National Fish Hatchery. See Paragraph No. 2 for specific project requirements.
b. Maximum Project Price: Reference the solicitation for the ‘Not to Exceed’ project budget. This number shall include all construction and demolition activities for the project.
c. PERIOD OF PERFORMANCE: See the solicitation /contract.
d. The FWS Engineering Project Manager for this project is John Loving. Any questions regarding the scope of work and technical matters shall be directed to John Loving, U.S. Fish and Wildlife Service, 1875 Century Blvd., Atlanta, GA 30345. Telephone: (404) 638-8481. Email: john_loving@fws.gov. See Section G of the solicitation /contract.
e. Government Furnished Information: See Section J of the solicitation /contract.
f. DEFINITIONS
i. Construction is defined as, but not limited to, constructing (giving form by combining the parts), altering, repairing, adding to, subtracting from, improving, restoring, moving, wrecking, or demolishing any building, highway, road, excavation or other structure, project, or improvement attached to real estate.
ii. Contracting Officer (CO) is defined as a person with the authority to enter, administer, and/or terminate contracts and make related determinations and findings on behalf of the United States of America. The term includes certain authorized representatives of the Contracting Officer such as the Construction Specialist (CS), Contracting Officer's Representative (COR), Construction Representative (CR) and Site Representative (SR) acting within the limits of their authority as delegated by the Contracting Officer.
iii. Day or Days shall mean calendar days unless otherwise specifically noted.
iv. Contractor is the contracting entity selected to design and construct the project.
v. “Service” or “FWS” means United States Department of Interior, Fish and Wildlife Service, Region 4 (Southeast Region).
vi. Government may also be referred to as the Owner, Contracting Officer, Contracting Officer's Representative, the Service, or Fish and Wildlife Service (FWS).
vii. Incident Rate (IR) is the number of recordable incidents resulting in days away from work for each 100 full-time employees per year, based on 2,000 hours worked per employee per year.
viii. Local Authority Having Jurisdiction: Duly adopted local code authority, agency, or serving utility governing or serving the location of the project.
ix. Notice to Proceed (NTP): There will be one NTP for this project issued by the Contracting Officer (CO) to initiate the construction phase.
x. Open Book Accounting is the right of the government to have free and open access to financial and accounting documents that the Contractor has associated with the project being designed and constructed.
xi. Public Works is construction performed under a contract with a Federal, State, County, City, or other government entity.
xii. Schedule is a chronological layout of the work to be performed prepared using commercially available scheduling software.
xiii. Subcontractor is any person or entity retained by Contractor as an independent contractor to perform a portion of the Work and shall include material, labor and suppliers.
xiv. Sub-Subcontractor is any person or entity retained by a Subcontractor as an independent contractor to perform any portion of a Subcontractor's Work and shall include material, labor, and suppliers.
xv. Work is comprised of all Contractor's permitting, construction and other services required by the Contract Documents, including procuring and furnishing all materials, equipment, services and labor reasonably inferable from the Contract Documents.
2. SPECIFIC PROJECT REQUIREMENTS
a. Remove the following (Include in Division 2 of Bid Schedule)
i. All abandoned lines and wires
ii. Furnace and ductwork
iii. Water Heater
iv. AC/Dehumidifier system in Lab and Toilet
v. Cabinets in Toilet
vi. Interior and Exterior Doors
vii. Overhead Garage Door
viii. Ventilators (Louvers and Fans)
ix. Main Area Sink
x. Gutters and Downspouts
b. Replace Toilet Cabinets (Include in Division 6 of Bid Schedule) Cabinets shall be constructed of ¾” thick plywood (no OSB). All surfaces shall receive one prime coat and one finish coat of semi-gloss paint.
c. Install Fiberglass Grating at Troughs (Include in Division 6 of Bid Schedule) Grating shall have 1 ½” square mesh with ¾’ depth as manufactured by McNichols of equal.
d. Install Spray Foam Insulation at underside of roof and gable ends (Division 7) Spray foam insulation shall be closed cell and applied to a minimum thickness of
4.5 inches to achieve an R-value of 30.
e. Install Fiber Cement Soffit (Division 7) Soffit shall be ¼” non-perforated/primed as manufactured by James Hardie or equal. Secure with stainless steel fasteners per manufacturer’s instructions. Soffit shall receive two coats of high-quality exterior latex paint.
f. Install new gutters and downspouts (Division 7) Install 6” K-Style gutters fabricated from minimum .032 gage aluminum. Secure hidden screw hangers with stainless steel screws coated to match gutter color. No gutter spikes or nails. Gutter shall have minimum 20-year warranty. Color to match existing metal roof or as selected by USFWS.
g. Install new Interior and Exterior Doors (Division 8) Sand/prime/paint existing hollow metal frames as needed. New doors shall be FRP (Fiber-reinforced plastic). Color as selected by USFWS.
h. Install new Overhead Door at each end of wet lab (Division 8) Overhead doors shall be as manufactured by C.H.I./Door Model 3241 or equal. Track type shall be standard lift. Track size shall be 2”. Horizontal and vertical gauge shall be 14. Color as selected by USFWS. Overall door dimensions shall conform to existing opening.
i. Install Gypsum Board Ceiling over Toilet and Lab (Division 9).
Gypsum board shall be ½” moisture-resistant with added mold resistance in the core and paper (“Purple Drywall”) as manufactured by Gold Bond or equal. Gypsum board shall receive two coats of latex-based waterproofing primer and one finish coat of high-quality exterior latex paint.
j. Paint and Seal Existing Roof Trusses (Division 9) Sand existing exposed roof framing as required. Apply two coats primer/sealer as manufactured by Kilz or equal. Apply two finish coats of high-quality exterior latex paint.
k. Paint existing concrete slab with Epoxy Coating (Division 9) Carefully prepare existing concrete slab prior to application of two-part epoxy coating as manufactured by Behr or equal.
l. Install new Oxygen Generator and Required Piping (Division 13) $10,000.00 allowance (See Bid Schedule).
m. Insulate main water lines (Division 22) Install minimum 1” thick foam tubular pipe insulation as manufactured by Frost-King or equal.
n. Replace City Water Hose bibbs (Division 22) Hose bibbs shall be approved by municipal authority prior to installation. Install according to all applicable municipal, state, and federal plumbing codes.
o. Replace City Water Main Control Valve (Division 22) Valve shall be approved by municipal authority prior to installation. Install according to all applicable municipal, state, and federal plumbing codes.
p. Replace main area sink (Division 22) Install new stainless-steel sink to match depth and dimensions of existing sink.
q. Install safety shower in toilet (Division 22) Safety shower shall meet all applicable OSHA requirements.
r. Install Eye-Wash station on sinks (Division 22) Eye-Wash stations shall meet all applicable OSHA requirements.
s. Install new AC/dehumidifier system in Lab and Toilet (Division 23) Contractor shall submit layout and all other appropriate information to USFWS for approval prior to installation.
t. Install new Ventilators (Louvers and Fans) (Division 23) Water Penetration Louvers shall be aluminum and offer maximum protection against rainwater. New louvers shall conform to existing dimensions in exterior masonry wall. Fans shall also be moisture/vapor resistant.
u. Install Dimmable LED ceiling fixtures with automatic timers (Division 24) Lighting fixtures, timers, switches, etc. shall be moisture/vapor resistant. Contractor shall submit proposed lighting layout for USFWS approval prior to installation.
v. Install Emergency Lighting (Division 24) Same requirements as referenced in item u. above
w. Detailed Electrical Requirements New wiring shall conform to all requirements of the National Electrical Code (NEC). Outlets shall be made moisture-resistant and/or weatherproof by using GFCI receptacles/weatherproof covers or by using any means and methods as allowed by the NEC. Contractor shall submit a wiring layout for USFWS approval prior to installation.
New three phase service panels – 225 amps main breaker
i. 110 V – 20-amp circuits
ii. 220V – 30-amp circuits
iii. 77 circuits
iv. 100-amp subpanel located on exterior wall
v. 3 – main lighting circuits (front, middle, back)
vi. Front light section switch at front door
vii. Lighting for each area switched from troughs
viii. Back light section switch at back door
ix. Wall outlets – 45 circuits
x. Every 10-ft span around outside walls
xi. 1 – 220V 30-amp circuit – switchable twist lock
xii. 1 – 110V 20-amp circuit – dedicated switchable outlet
xiii. 1 – 110V 20-amp circuit – 3 duplex receptacles spaced every 3 feet
xiv. Trough outlets – 17 circuits
xv. 110V 20-amp circuit – 4 duplex receptacles
xvi. Back center area
xvii. Add additional - 2 circuits with 4 duplex receptacles each
xviii. 2 circuits – 220V 30-amp circuit – switchable twist lock 2 circuits – 110V 20-amp circuit – dedicated switchable outlet
xix. Toilet – 5 circuits
xx. 110V 20-amp circuits – hand dryer, refrigerator, freezer, wall outlets, mini-split
xxi. Lab – 2 circuits
xxii. 110V 20-amp circuit – mini-split, wall receptacles
xxiii. Ice Machine - 1 circuit - 110V 20-amp circuit
xxiv. Blower - 1 circuit - 110V 20-amp circuit
xxv. Cord reels down middle - 3 circuits - 110V 20-amp circuit
3. ADDITIONAL PROJECT REQUIREMENTS
a. Possess and maintain, for the duration of this project, a state license as a general contractor in the State of Georgia, if required by the state.
b. Provide supervision, labor, materials, equipment, tools, construction equipment and machinery, inspection, testing, start-up, water, heat, utilities, transportation, and other facilities and services necessary for proper execution and completion of the Work, whether temporary or permanent and whether incorporated or to be incorporated in the Work.
c. Have at least one qualified supervisor capable of reading, writing, and conversing fluently in the English language on the job site during working hours.
d. Perform all construction activities efficiently and with the requisite expertise, skill, and competence to satisfy the requirements of the Contract Documents. At all times exercise complete and exclusive control over the means, methods, sequences, and techniques of construction.
e. Utilize Subcontractors who are duly licensed and qualified to perform the Work consistent with the Contract Documents. Government may reasonably object to Contractor's selection of any Subcontractor.
f. Contractor is responsible for quality control of its own work and work performed by its subcontractors and their sub-subcontractors.
g. Assume responsibility for the proper performance of the Work of Subcontractors and all acts and omissions in connection with such performance.
h. Coordinate the activities of all Subcontractors. If Government performs other work on the Project or at the Site with separate contractors under Government's control, Contractor agrees to reasonably cooperate and coordinate its activities with those of such separate contractors so that the Project can be completed in an orderly and coordinated manner without unreasonable disruption.
i. All work shall be performed in close coordination with Hatchery Manager.
j. Minimize creation of construction, deconstruction, and demolition waste. Minimize factors that contribute to waste such as over-packaging, improper storage, ordering error, poor planning, breakage, mishandling, and contamination. Divert 50%, minimum, of total project waste from landfills. Dispose of all materials not salvaged, reused, or recycled legally, off-site, in licensed disposal facilities. The Contractor shall coordinate with the Hatchery on an appropriate location for demolished materials if required. Such materials become the Contractor’s property and shall be removed from the site as soon as possible. In addition, Contractor shall coordinate with Hatchery a construction staging area for storage of all required construction materials.
k. During normal working hours throughout the duration of the contract, promptly furnish information and provide reasonable facilities and access for obtaining information respecting: the character of the materials, condition of the work-in-progress, number of workers employed, their pay rate, time worked, inspection reports and test results from sampling and testing laboratories, and similar information.
l. Government buildings and other facilities are not available for storage of Contractor's materials, equipment, and tools except for designated staging areas. Government owned equipment is not available for Contractor's use to accomplish any Work.
m. Furnish all temporary facilities and utilities needed including heat, light, power, water, telephone, sanitary facilities, job offices, material handling equipment, temporary construction aids, and storage enclosures. The Contractor is responsible for all temporary connections and disconnections. Government furnished electricity is not available for Contractor’s use.
n. Communication:
1. RFI Process: The Contractor shall utilize the following protocol:
a. Utilize a RFI Form: All supplemental information and answers to questions shall be documented on Request for Information (RFI) forms. The Contractor may use any format they choose for the Request for Information form but all shall be identified with the words “Request for Information” at the top of the form along with the Government Contract Number, Project Location (Warm Springs NFH) Project Title (Wet Lab Phase 2) and the Contractor’s Name.
b. Numbering: RFI’s shall be sequentially numbered, logged and filed and shall be part of the Closeout Documents submitted to the Government.
c. FWS Concurrence: As RFI’s are generated and answered, submit copies to the COR for review within 2 business days for concurrence via email .pdf format. A non-response from the COR within 4 days of receipt of the RFI will signify concurrence with the RFI answer. If the RFI is sent on a Friday, then Saturday and Sunday do not count as business days.
d. RFI Files Onsite: The Site Superintendent shall maintain a file of RFI changes. This RFI file shall be subject to inspection by the FWS without advance notice.
o. Submittals: The Contractor shall utilize the following protocol:
| 1. Utilize a Submittal Register: Develop a submittal register | keyed to the Drawings and Specifications and update this | Register as submittals and logged and processed. | ||||||||||||||
| 2. Numbering: Submittals shall be sequentially numbered. | A completed submittal register shall be submitted with the | Project Closeout Documents. | ||||||||||||||
| 3. FWS Concurrence: Provide the FWS Engineering Project | Manager with (1) copy of the approved submittal and | provide the FWS Construction Representative (1) copy of | the approved submittal within (2) business days of the | approval date. | ||||||||||||
| 4. Site Responsibility: The Contractor’s Site Superintendent | shall maintain a file of approved submittals for reference | purposes. This Submittal file shall be subject to inspection | by the FWS without advance notice. | |||||||||||||
| p. Any delays resulting from FWS questions/stoppages due to a lack of, or | inordinately lengthy or unnecessary delays in, the flow of information from the | Contractor to the Service for a time period in excess of 21 days, will not give rise | to claims for monetary consideration under the contract, including but not limited | to requests for equitable adjustments, constructive changes, and other claims for | compensation. | |||||||||||
| q. Payment will be made monthly for approved work performed in accordance with | the contract requirements under the item listed in the Schedule of Items and | based on the approved Schedule of Values. Schedule of Values: Prepare a | Schedule of Values in the format of the American Institute of Architects (AIA) | form number G702/703 Application and Certificate for Payment (or approved | equivalent format). Submit a detailed Schedule of Values no less than seven | calendar days prior to the start of on-site construction work. | ||||||||||
| r. The Contractor shall always have the ultimate responsibility for safety on the | project site, from issuance of the Notice to Proceed until final completion and | acceptance of the project by the Contracting Officer. Ensure that all onsite | activities, equipment, and facilities constructed by the Contractor, | subcontractor, or supplier conform fully to the standards of the Department of | Labor, Occupational Safety and Health Administration (OSHA) 29 CFR 1926 | and 1910; and DOI and FWS policies. In addition, the Contractor shall adhere to | all current Federal, State, and local safety requirements. In the event where any | safety standard referenced herein conflicts with another, the more stringent shall | govern. See Section H of the solicitation /contract. | |||||||
| s. Initiate and maintain, throughout the performance of this contract, an effective | safety and health program that provides adequate policies, procedures, and | practices to protect employees from, and allow them to recognize, job-related | safety and health hazards. The program shall include provisions for the | identification, evaluation, prevention and control of general work site hazards, | specific job hazards, and potential hazards that may arise from foreseeable | construction methods and conditions, as well as providing a competent person to | conduct frequent and regular inspections. Each employee (Contractor and | subcontractors) must be instructed in the recognition and avoidance of unsafe | conditions and the regulations applicable to the work environment. |
t. Work Hours: See Section H of the solicitation /contract.
| u. Comply with all applicable federal, state, and local environmental laws and | regulations. | ||||||||||||
| v. Thoroughly familiarize the local FWS staff with the improvements and train them | to be proficient in overall system operation and maintenance. Deliver to | the project site all specialty tools, valve handles, fan belts, filters, spare part, and | similar devices such that the FWS staff is able to operate and maintain the ‘ | work. Deliver maintenance materials to Station Manager, coordinate location of | storage. (Provide consumable items in such quantities as needed for the first | year of operation.) | |||||||
| w. Final Inspection: Submit a request in writing to the CO requesting the Final | Inspection. Notify the COR at least 10 calendar days prior to the anticipated date | of completion of all work. The COR will proceed with Final Inspection as promptly | as practicable. The time required for such Final Inspection and for making any | corrections as a result thereof shall be included in the contract performance time. | The Contractor representative responsible for the work shall be present at the | Final Inspection and shall submit a preliminary list of unfinished work prior to Final | Inspection. See Section E of the solicitation /contract. | ||||||
| x. Closeout Documents - Operation and Maintenance Manuals: Submit 3 copies of | Operation and Maintenance manual (two to the FWS field station and one to the | Regional Office Engineering) for the mechanical and electrical equipment, | architectural treatments, doors, and windows. The manuals shall be clear, well | organized, clearly indexed, tabbed, highlighted, tailored to the specific make, | model, and wiring configuration of the systems; and shall include | schedules and instructions for routine operation, trouble shooting, and | periodic maintenance. The manuals shall include complete parts lists keyed | to suppliers (name, address, websites, and telephone numbers for each). |
y. Permits, Warranties & Other Required Documents:
| i. Operational Permits: Provide copies of all approved operational permits. | Include a list of all monitoring requirements and monitoring frequencies. | ||||||
| ii. Submit fully executed manufacturer's standard material, product, and | equipment warranties. | ||||||
| iii. Submit any requested regulatory-agency clearance documents indicating the | work complies with their requirements such as final plumbing/gas/electrical | inspections. | |||||
| iv. After completion of the work and prior to final payment, submit a Release of | Claims (Form DI-137 provided by the Contracting Officer), properly executed | by the Contractor, releasing claims against the United States arising out of this | contract, other than claims specifically excepted from the operation of the | release. See Section I of the solicitation /contract. |
4. GOVERNMENT RESPONSIBILITIES
a. Government will, throughout the performance of the Work, cooperate with Contractor and perform its responsibilities, obligations, and services in a timely manner to facilitate Contractor's timely and efficient performance of the Work and to not delay or interfere with Contractor's performance of its obligations under the Contract Documents.
b. Separate Contractors: Government is responsible for all work performed at the Site by separate contractors, if any, under their control. Government will contractually require its separate contractors to cooperate with each other.
