STATEMENT OF WORK

T’siya (Zia) Day School 1000 Borrego Canyon Road Zia Pueblo, NM 87053 Demo and Install Touchless Faucets, Toilets, Urinals, and Filtered Water Bottle Filling Stations

1. SCOPE OF WORK:

The contractor shall demo 23 sink faucets (restroom), 4 sink faucets (Break Room, Nurse’s Office, Home Ec Room, & Science Room), flush valves for 25-toilets and 5-urinals, and install new touchless faucets, automatic/sensor flushometers for toilets and urinals, and 7-filtered water bottle filling stations at each water fountain will need to tie into a GFCI outlet. Contractor shall be responsible to dispose of demo’d parts and materials. After installation of new faucets, flushometers, and filtered water bottle filling stations, contractor shall be responsible to do patch work and check for operation.

2. PROJECT LOCATION:

3. PURPOSE:

The purpose of the project is to repair replace all manual turning and flushing of sinks, toilets, and urinals; to also include filtered water bottle filling station, minimizing spread of germs.

4. SPECIFIC REQUIREMENTS:

A. SCHOOL SITE VISIT:

Not required.

B. SERVICES:

The contractor shall perform the following:

· Demo 23 sink faucets (restroom); install new touchless faucets.

· Demo 2 sink faucets (Break Room & Nurse’s Office); install new touchless faucets.

· Demo flush valves for 25-toilets and 5-urinals; and install new touchless faucets, automatic/sensor flushometers for toilets & urinals.

· Will be responsible to dispose of demo’d parts and materials after installation of new faucets, flushometers, and filtered water bottle filling stations.

· If backing is required during installation, trim and finish shall be done when complete.

· Check for operation upon completion.

C. GENERAL REQUIREMENTS:

a. QUOTE/PROPOSAL:

Quotes/Proposal shall be itemized by material, supplies, equipment, labor costs, and NM State Taxes.

b. WORK HOURS:

Work hours for this project shall be schedule with school maintenance worker.

c. SAFETY:

The contractor shall adhere to all BIA/BIE adopted codes and the contractor shall abide by all OSHA regulations while on school grounds and shall review all applicable safety regulations before beginning the project. The contractor shall provide signs, barriers, and or caution tape as a safety precaution around the work site. The contractor shall have a fall protection plan if workers are to be performing duties above six (6) feet from the ground or finished floor. The contractor shall bear sole responsibility of taking all appropriate actions necessary to prevent accidents, injuries to individuals and damage to property in and around the work area.

d. BACKGROUND INVESTIGATION:

The awarded contractor shall provide a designated individual(s) from their company who will be subject to a BIA Security Background Investigations in accordance with Section 231 of the Crime Control Act of 1990, Public Law 101-647. Background Investigation forms will be provided at the pre-construction meeting.

e. ACCESS:

BIE Facility Management personnel shall provide access to T’siya Day School at all times. The contractor shall minimize disruption of classroom instructions and normal school activities expected in such areas.

f. DAMAGES:

Where existing surfaces are damaged the contractor shall repair and finish with matching existing structure. The contractor shall be responsible for replacement of any items (interior or exterior) damaged during the performance of this project.

g. CLEANING:

The Contractor shall keep the work area reasonably clean at all times. Contractor shall accomplish a complete cleanup at the end of the project. The Contractor shall properly dispose of all debris at an approved dumping site.

h. PERFORMANCE:

Performance time for this project is 90 Calendar Days from Notice to Proceed. (Exception: Workdays maybe interrupted with closures of the Pueblo due to Tribal Traditional Activities). All work shall be completed in a professional manner and without delay once the on-site work is started.

i. SALVAGE:

All salvageable materials and equipment shall be the property of the contractor unless otherwise noted in the Statement of Work and/or drawings. The Contractor is responsible for the removal of all salvageable materials and equipment from the installation.

5. PRODUCTS:

A. WARRANTY:

The Contractor shall provide a written warranty on the material and workmanship from date of acceptance by the Government.

B. MATERIALS:

The Contractor shall provide a written statement noting all material being utilized are asbestos free.
