About this file
This is a Scope of Work document for an RFQ issued by the Nevada Department of Employment, Training and Rehabilitation, Bureau of Vocational Rehabilitation, seeking qualified vendors to provide vehicle modification and conversion services for vocational rehabilitation clients throughout Nevada. The requested services include structural vehicle modifications, installation of mobility equipment and adaptive driving devices, high-tech driving systems installation, equipment adjustments, and repair services. Vendors must be accredited for all work types they perform and employ certified technicians for equipment they sell, install, and service. Prior to service delivery, vendors must assess vehicle structural capacity, conduct inspections on vehicles three years old or older or with 50,000+ miles to document condition and reliability, and provide written cost estimates for approval. Upon completion, all modifications must meet factory specifications and Federal Motor Vehicle Safety Standards, with vendors required to sign an Installer Affidavit of Completion and submit detailed invoices with supporting documentation. The RFQ specifically identifies Ability Center and Mobile Works as current service agreement holders to receive this solicitation.
Vendors must submit detailed proposals including company background, accreditation status with the National Mobility Equipment Dealers Association's Quality Assurance Program (though not required for award consideration), vehicle transportation capabilities, detailed service descriptions, geographical service locations, number of certified technicians, and a minimum of three references preferably from governmental entities. Parts and materials may be invoiced at no more than 20% markup above vendor cost, with the state reserving the right to request invoices for verification. Vehicle inspection may be conducted by an independent vendor at the state's discretion, though the modification vendor remains responsible for notifying DETR staff of required repairs or reasons why modifications may not be warranted. Payment will not be issued without all required documentation, and vendors do not need to qualify for all listed services or provide statewide coverage to be considered for award.

Modifications/conversions include structural vehicle modifications; installation of mobility equipment and devices including customized and advanced adaptive driving equipment and controls; installation of high-tech driving systems; adjustments to fit required equipment to specific needs and equipment repair services as needed.
Scope of Work:

Vendors must provide a detailed description of their proposed services, which may include those listed on the attached Vehicle Modification and Conversion List of Services. Vendors must be accredited for all of the types of work they perform and must have certified technicians in the equipment they sell, install and service. Vendors must return a completed Vehicle Modification and Conversion List of Services form and submit copies of all applicable licenses and/or certifications. Vocational Rehabilitation clients are located throughout the State of Nevada. Vendors must provide a detailed list of all their Geographical Service Location(s) where the proposed services will be performed. Vendors do not need to provide statewide service locations to be considered eligible for award.
Prior to performing requested services, Vendor must:
· Assess whether the vehicle can structurally support the requested services. Already modified equipment must be included in this assessment.
· If the vehicle is three (3) years old or more; or has 50,000 miles or more, inspect the vehicle to determine its condition and including the reliability of the engine and/or other appropriate systems such as the power train, brakes, electrical system, body and safety features. The results of this inspection must be documented and the Vendor must provide a written certificate that the items have been inspected, are in good condition and the vehicle warrants requested services.
· DETR reserves the right to have the vehicle inspection performed by an independent vendor. This does not remove the vehicle modification vendor’s responsibility to notify DETR staff of any noticeable repairs required or reasons why a modification may not be warranted for the vehicle.
· Due to the nature and extent of work varying for each vehicle, Vendor must provide a written cost estimate/quote based on each vehicle modification and conversion requirements, detailing labor and parts/material costs for prior approval. Cost estimate/quote shall be submitted to the Rehabilitation Administrator or designee.
· Parts/Materials shall be invoiced at no more than 20% markup above Vendor’s cost. The state reserves the right to request copies of the Vendor’s parts and material invoices to verify.
Upon completion of requested services, Vendor must:
· Any modifications, installations, adjustments and repairs must meet factory specifications and conform to the Federal Motor Vehicle Safety Standards (FMVSS).
· If it is anticipated that the vehicle will not conform with the FMVSS after requested services are completed, Vendor must have an exemption from the “make inoperative” prohibition mandated through the National Highway Traffic Safety Administration (NHTSA) (49 CFR Part 595).
· Sign and date an Installer Affidavit of Completion of Vehicle Modification VM-5 Form (to be provided by the Rehabilitation Division) certifying that the work has been completed in compliance with the prescription and/or specifications and written warranties have been provided to the participant.
