About this file
This is a Past Performance Questionnaire for the Manor Independent School District (MISD) in Texas, designed to evaluate contractor performance for RFP 26-011RFP, MISD Lawn Maintenance Services. The questionnaire must be completed independently by offeror client references and submitted directly to Chrissie Bryant, Purchasing Coordinator, at purchasing@manorisd.net by Monday, June 15, 2026 at 2:00 PM Central Time. Offerors are required to provide at least three preferred references from current customers outside of Manor ISD, with emphasis on Educational Agencies (K-12 preferred) and Government Agencies substantially serviced by the proposer. The lawn maintenance services contract encompasses mowing, trimming, blowing, and weed control treatments for nine MISD campuses, with service scheduled for every two weeks from July through December (13 visits per site). Submission of the questionnaire must occur on company letterhead via email from a corporate email account, with offerors responsible for ensuring their references complete and submit the forms independently.
The questionnaire requests detailed information regarding the contractor's past or current reference contract, including contract number, value, period of performance, and a description of items provided. References must rate the proposer across multiple performance dimensions using standardized evaluation criteria: Exceptional, Very Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable. Evaluation areas include service type delivery, work timing, quality of services and goods, meeting of expectations, timeliness of completion, and recommendation for future work. References are instructed to provide explanatory narratives where possible to support their ratings, with additional pages available for expanded commentary. The questionnaire serves as a critical component of the overall proposal evaluation process for determining contractor capability and historical performance reliability in providing similar services to school districts and government entities.
