About this file
This is a Pre-Award Risk Questionnaire for subrecipients seeking funding under the American Rescue Plan of 2021 (ARPA) administered by Harris County, Texas, in compliance with the Department of the Treasury and 31 CFR 35 requirements. The questionnaire is designed for informational purposes and must be completed by all subrecipients before entering into a subagreement with the County for work under the ARPA grant. The document functions as a comprehensive risk assessment tool to assist County program staff in determining appropriate conditions and assistance provisions for subagreements. Completion of this questionnaire combined with the County's internal program staff assessment will inform award decisions and subagreement terms.
The questionnaire requires subrecipients to provide detailed information across multiple risk categories, including performance history of federal awards, management systems and personnel capabilities, financial stability, audit history, and internal controls. Organizations must disclose federal funding received in the preceding five years, current funding percentages from federal sources, changes to key management positions and systems, capacity to implement federal regulations including 2 CFR 200, existing program experience, and executive compensation information for the five highest-paid officers. Additionally, subrecipients must document their accounting systems, ability to segregate expenditures by funding source, and status of any unresolved audit or monitoring findings. There are no predetermined answers that automatically disqualify an organization, and applicants may request technical assistance if lacking experience with applicable federal compliance requirements.
