PAST PERFORMANCE QUESTIONNAIRE

The purpose of this questionnaire is to obtain information regarding the above named Contractor’s past/current performance relative to a contract completed, or in progress, at your company/agency.
NASA Stennis Space Center (SSC) is considering this Contractor for a task order for construction services at SSC. We would greatly appreciate your input regarding the quality of the Contractor’s past performance.
Handwritten responses are sufficient. We request that the individual(s) responsible for oversight of the project (i.e., Contracting Officer, Contracting Officer’s Representative, etc.) respond to the questionnaire.
Upon completion of the questionnaire, please email the questionnaire to Ms. Rachel Clayton, at rachel.clayton@nasa.gov. If you have any questions please call (228) 688-2822.

2. Relevant (Pertinent) Experience: The accomplishment of administrative support services similar in size, scope, and complexity to the administrative support services required by this solicitation. Relevant/pertinent size is defined as projects over $4M.
Relevant/pertinent scope is defined as administrative services. Providing past administrative services experience that includes administrative services for executive level personnel, travel coordination, and safeguarding of sensitive information will receive additional consideration. Relevant/pertinent complexity is defined as administrative services performed at multiple geographic locations and a continuous employee-training program. Providing past experience that includes services at multiple geographic locations under a single contract and a continuous employee-training program will receive additional consideration.

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE
RELEASE OF PAST AND PRESENT PERFORMANCE INFORMATION TO THE
PRIME CONTRACTOR
Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor’s or teaming partner’s consent.
Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the Government in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past Performance Volume (or Proposal). (Note: Section 5 should specify if past performance is to be submitted as a separate proposal volume.)
