This document is a Federal Tax Liability and Recent Felony Convictions certification form for Madison County Transit's ACT 403(b) Employee Education Services contract (No. 25-2-13601). The form is a mandatory certification required for vendors bidding on contracts exceeding $10,000, specifically addressing federal tax and criminal history requirements. Vendors must certify that they have no outstanding federal tax liability that has exhausted all judicial and administrative remedies and that they have not been convicted of a federal felony within the preceding 24 months.
The certification applies to private corporations, partnerships, trusts, joint-stock companies, sole proprietorships, and other business associations. It requires the bidding entity to affirm two key conditions: (a) there are no unpaid federal tax liabilities that have been assessed and are not being paid according to an agreement with tax collection authorities, and (b) the entity has not been convicted of any federal felony criminal violations in the past 24 months. The requirement extends to all subcontracts at every tier, ensuring comprehensive compliance across the entire contract execution process.
