About this file

The document is a Minority Business Enterprise (MBE) Form for the City of Fort Myers, Florida, specifically related to the RFP Fleet GPS project for providing GPS services for fleet vehicles. The form is designed to document a contractor's good faith efforts in meeting the city's MBE goal of 15%, as established by Resolution 38-125. The form requires detailed documentation of potential minority business enterprise (MBE) subcontractors, including contact information, contract amounts, work descriptions, and efforts to engage MBE firms in the bidding process.

The form provides a structured approach for prime contractors to demonstrate their commitment to minority business participation, with six key steps: prime contractor information, MBE contractor details, record of transmittal letters, telephone/in-person visit documentation, handling of incomplete quotations, and explanation of non-utilized MBE firms. The form is intended to be comprehensive, requiring contractors to provide specific details about outreach efforts, communication methods, and reasons for contractor selection or non-selection, ultimately supporting the city's objectives of promoting diversity and inclusion in municipal contracting opportunities.
