About this file

The document is a Minority Business Enterprise (MBE) Form from the City of Fort Myers, Florida, designed to support the city's MBE goal of 15% for the SCADA On Call Continuing Services contract. The form provides a structured template for prime contractors and subcontractors to document their good faith efforts in obtaining MBE participation, with sections for contractor information, MBE contractor details, record of transmittal letters, telephone/in-person visit records, and handling of incomplete or non-responsive quotations.

The form requires detailed documentation of MBE engagement, including specific information such as total contract amount, MBE dollar amount, labor/service costs, materials costs, contract percentage, project commitment and completion dates, and verification of communication efforts with MBE firms. The goal is to ensure transparent and equitable participation of minority-owned businesses in the city's contracting process, with contractors expected to provide evidence of their outreach and consideration of MBE firms through transmittal letters, contact records, and explanations for any MBE quotes not utilized.
