About this file
This document is a Scope of Work for the Utah Department of Corrections (UDC) related to licensing and utilizing the Level of Service (LS) family of assessment instruments. The contract provides UDC with the ability to use three assessment tests: the Level of Service/Case Management Inventory (LS/CMI), the Level of Service/Risk, Need, Responsivity (LS/RNR), and the Level of Service-Revised: Screening Version (LS-R:SV). The contract allows UDC to install the Software Development Kit, administer the assessments to offenders, enter assessment scores into their Offender Tracking system, and receive updates to the instruments. The contract term appears to be five years, with pricing increasing incrementally each year.
The compensation structure is based on per-use pricing for various components of the assessment instruments, including permission to copy reports, scoring guides, and manuals. Prices range from $2.00 to $3.00 for report use and up to $137.00 for hard copy manuals in the fifth year. The contract includes strict confidentiality provisions, intellectual property protections, and security measures, including background checks for contractor staff. Funding is based on average usage, with the potential for increased funding through contract amendments if demand exceeds initial projections. The contract is being awarded through a sole-source procurement process, with the justification that there is only one source for these specific assessment instruments.

SCOPE OF WORK
I. GOALS AND OBJECTIVES
It is the goal of Adult Probation and Parole to maintain accurate, up to date information on the factors influencing each supervised offender’s likelihood to recidivate. This improves the ability of UDC staff to manage offenders and ultimately supports the mission of UDC:
UDC Mission “Our dedicated team of professionals ensures public safety by effectively managing offenders while maintaining close collaboration with partner agencies and the community. Our team is devoted to providing maximum opportunities for offenders to make lasting changes through accountability, treatment, education and positive reinforcement within a safe environment.”
II. PERFORMANCE MEASURES
CONTRACTOR will perform in accordance with the following specifications. Quality of service and performance will be measured by response time, record keeping, and invoicing in accordance with contract requirements, and by measuring and evaluating evidence provided by the CONTRACTOR and monitoring of the CONTRACTOR’S compliance with every term and condition as defined in Attachments A through E.
III. DETAILED SCOPE OF PERFORMANCE
This contract provides UDC with the ability to utilize three assessment tests commonly known as the LSI family of instruments: the Level of Service/Case Management Inventory (LS/CMI); the Level of Service/Risk, Need, Responsivity (LS/RNR); and the Level of Service-Revised: Screening Version (LS-R:SV). The rights granted in this contract shall not constitute a sale of all or part of the LS/CMI, LS/RNR, and/or the LS-R:SV.
A. LICENSES
