Version 11-30-2016-1 FAR Part 11—Describing Agency Needs

PURCHASE DESCRIPTION

1. Statement of Need and Purpose:

The Clinical Research Unit requires an integrated, validated system capable of quantifying both macrocirculatory function (ankle-brachial index, toe-brachial index, segmental limb pressures, and pulse volume recordings) and microcirculatory function (transcutaneous oxygen tension, laser Doppler skin perfusion, and skin perfusion pressure) within a single standardized clinical research visit. No equipment currently available to the Clinical Research Unit meets this combined macro/microvascular assessment need in one platform; assessments are presently fragmented across disparate devices, which limits throughput, standardization, and the ability to correlate macro- and micro-level vascular findings within the same participant encounter.

The purpose of this acquisition is to equip the NIA Clinical Research Unit with a combined macro-and microvascular diagnostic platform to enable standardized, protocol-driven vascular assessment in BLSA and GESTALT participants, support direct within-subject comparison of macro- and microvascular status, and generate longitudinal vascular biomarker data linkable to the broader BLSA/GESTALT datasets. Without this system, the Clinical Research Unit will continue to rely on fragmented, non-integrated equipment, constraining the scientific scope of BLSA and GESTALT in characterizing vascular contributions to aging phenotypes and delaying the generation of combined macro/microvascular biomarker datasets that are increasingly central to aging and geriatric vascular research.

2. Background Information and Objective:

The Clinical Research Unit of the National Institute on Aging (NIA) conducts two major longitudinal studies of human aging — the Baltimore Longitudinal Study of Aging (BLSA) and GESTALT (Genetic and Epigenetic Signatures of Translational Aging Laboratory Testing). Both studies require comprehensive, repeatable, non-invasive physiological phenotyping of participants across the adult lifespan in order to characterize the trajectories of biological aging and identify early biomarkers of age-related disease.

Vascular health is a central determinant of healthy aging. Both macrovascular disease (peripheral arterial disease, critical limb ischemia) and microvascular dysfunction (impaired skin and tissue perfusion, reduced microcirculatory reserve) are known to progress silently with age and are independently associated with frailty, impaired wound healing, cardiovascular morbidity, and mortality. Capturing both levels of circulatory function — macrocirculatory (large vessel) and microcirculatory (capillary-level perfusion and oxygenation) — in the same cohort of participants over time is essential to understanding how vascular aging contributes to the broader aging phenotype studied by BLSA and GESTALT.

Peripheral vascular dysfunction is both a consequence and an accelerant of aging-related decline. Macrovascular disease (e.g., PAD) restricts blood flow to the limbs and is linked to impaired mobility and increased cardiovascular risk, while microvascular dysfunction impairs tissue oxygen delivery and wound healing, and is increasingly recognized as an early, sensitive marker of systemic vascular aging — often preceding overt macrovascular disease.

By incorporating combined macro/microvascular phenotyping into BLSA and GESTALT, the CRU will be positioned to:

• Characterize the natural history and rate of progression of both macro- and microvascular aging in a well-characterized, longitudinally followed cohort.

Version 11-30-2016-1 FAR Part 11—Describing Agency Needs

• Investigate whether microvascular measures (tcpO2, laser Doppler perfusion, SPP) serve as earlier or more sensitive indicators of vascular aging than traditional macrovascular measures (ABI/TBI) alone.

• Examine associations between vascular function and other BLSA/GESTALT domains, including physical performance, body composition, inflammatory markers, and epigenetic aging clocks.

• Support investigations relevant to wound healing capacity, frailty, and functional decline

— outcomes of direct relevance to NIA's mission.
