FAR 52.212-1, “Instructions to Offerors–Commercial Items”
Quote Preparation Instructions
All quoters shall submit the following:
(1) The response should consist of three (3) volumes. The volumes are: Volume I – Technical Submission, Volume II – Past Performance, and Volume III – Price.
(2) Quote must comply with the page limitations and format specified for each volume. Information submitted beyond limitations identified could negatively impact the evaluation during the rating process. The follow-on paragraphs provide the specific information required for each volume.

(3) Additional format requirements for quotes to Volumes I, II and III must be as follows:
· Elaborate formats, bindings or color presentations are not desired or required.
· Charts, Graphs, Tabs, Diagrams, Tables, Photographs, Drawings, Title Page, Table of Contents, Covers for Volumes, Glossary of Terms/Abbreviations and Acronyms, Indices, section dividers and job qualifications (Resumes) are excluded from the page limitation.
· All volumes must be clearly marked as either Volume I - Technical Submission, Volume II - Past Performance, or Volume III – Price, respectively.
(4) Quote Content:
a. VOLUME I – TECHNICAL CAPABILITY: The Technical Capability volume shall be clear, concise, and include sufficient detail for effective evaluation and for substantiating the validity of stated claims. Legibility, clarity and coherence are very important. The Technical volume shall be organized according to the following general outline:
(a) Tab 1 – Management Approach: Provide your detailed management approach to satisfying the requirements of the contract. In addition to your approach, include the following:
· A detailed organizational chart with management roles and responsibilities;
· Provide a detailed narrative describing your process for managing and controlling team members in multiple functional areas, to include subcontractors
(b) Tab 2 – Technical Approach/Experience, Teaming and Staffing Approach (for work under Attachment I – General SOW):
· Provide your detailed technical approach to satisfying the requirements of the contract. This must include a narrative describing your overall contract strategy proposed for satisfying all the activities listed in the SOW.
· Team Structure: Describe your team structure (to include utilization of subcontractors), the work/work area allocated to each team member. Team structure must consist of a faculty of qualified instructor personnel. Instructors shall be knowledgeable in various facets of leadership and have expertise in presenting and facilitating the program content. Additionally, instructors shall be knowledgeable with federal management issues and familiar with the business of science.
· Provide a list of labor categories deemed necessary to perform under the contract.
· By name resumes associated with each key labor category should be submitted to demonstrate the knowledge, skills, training, and education levels for this labor category considered necessary for each key labor category proposed (Simply claiming the proposed qualifications is not acceptable). Executive Coaches should have, at a minimum ICF certification. At least one-third of the coaches should have a background in coaching scientists. NIEHS reserves the right to review all coaches and instructors prior to selection.
· Provide specific evidence of your experience administering, evaluating, and coaching with a variety of assessments and instruments for relevant metrics on team performance.
· Availability of the proposed personnel appointed to this contract effort.
(c) Tab 3 – Response to Task Order SOW (RFQ Attachment II): An actual task order is going to be awarded as a result of your response. Provide your response to satisfying the specific task order SOW provided. In addition to your approach, include the following:
· Provide a draft detailed Plan of Instructions (POI) with a proposed detailed schedule of the training program you intend to use to successfully execute the task order SOW. The POI and schedule will require final approval prior to performance beginning on the task order.
· Provide a list of labor categories and their associated labor hours deemed necessary to perform the task order.
b. Volume II – Past Performance: The Past Performance volume shall be clear, concise, and include sufficient detail to demonstrate the Quoter possesses the relevant technical experience required to successfully perform the contract requirements. The Quoter shall submit the following information:
(a) Tab 1 – Reference List: The Quoter shall provide a minimum of three (3) references.
· A list of three (3) references with who like or similar services has been provided within the past three (3) years. The past performance references must include contract number, dollar value, specific points of contact, phone number and email address. Contracts listed may include those entered into by the Federal Government, agencies of state and local governments and commercial concerns.
· Quoter may also submit past performance information regarding predecessor companies, key personnel who have relevant experience or subcontractors that will perform major or critical aspects of the requirement when such information is relevant to the instant acquisition. For the purposes of this solicitation, a "major subcontract" is defined as supplier, distributor, vendor, or firm that furnishes supplies or services to or for a prime contractor.
(b) Tab 2 – Reference Information: The Quoter shall include for each reference:
· Project Title
· Description of the project
· Contract and, if applicable, task order number
· Total Contract Value and type of contract
· Government Agency/Organization
· Contracting Officer’s Name and Telephone Number.
· Current status, e.g. completed and/or if in progress start and estimated completion dates.
· Name of company being referenced
· SOW paragraphs that the reference applies to Key personnel (please highlight those individuals who worked on the relevant project (s) and are being proposed for this effort).
· A brief narrative of why you deem the reference to be relevant to this effort.
c. Volume III – Price Submission: The price submission must be clearly marked “Volume III, Price Submission”. The price volume shall be organized according to the following general outline:
(a) Tab 1 - RFQ Document. The contractor must submit the following documents as part of the RFQ Document:
1. Point of Contact: The Quoter shall provide the name, title and telephone number of the company/division point of contact regarding decisions made with respect to your quote and who can obligate your company contractually.
2. Cover Letter: This statement page shall also include the following information:
· Company Name/Address: DUNS #:
· Cage Code:
· Federal Tax ID:
· Point of Contact:
· POC Phone/Fax/E-mail:
3. Representation, Certifications, and Other Statements of Quoter – In accordance with provisions FAR 52.212-3, Offeror Representations and Certifications-- Commercial Items, IF YOU INTEND TO SUBMIT A QUOTE, YOU MUST: (1) Go to the Online Representations and Certifications Application (ORCA) and complete the Representations and Certifications. Offerors must access ORCA through the System for Award Management (SAM) at http:// www.sam.gov; and (2). Complete, and INCLUDE as part of your price quote.
(b) Tab 2 – Price Quote. The contractor shall submit the following documents as part of the Price Quote:
1. Contract Pricing Matrix: The Quoter shall complete the pricing matrix below for hourly rates that will be incorporated into the resultant contract. The matrix will be used in the pricing future task orders issued under the contract. The price matrix will have specific rates that reflect the maximum rates to be paid for listed items through the life of the contract.

2. Task Order Pricing Matrix: Quoters shall submit separate pricing for STO 1. Quoters shall provide a corresponding table that includes labor categories, hours, and hourly rate for the TO. Pricing for the TO shall include fully burdened labor rates, G&A, OH, fringe, and profit.
1. Task Order Price Sheet : The Quoter shall complete the matrix below to price the TO Technical Solution. Pricing for the STO shall the total price to successfully execute and complete all the tasks required. Hourly rates must be fully burdened to include G&A, OH, fringe, and profit.

4. The Quoter agrees to hold the prices in its offer firm for 120 days from the date specified for receipt of offers.
5. Quoter shall submit only one offer presenting the terms and conditions or commercial items for satisfying the requirements of this solicitation.
