About this file

This document is an amendment to a Request for Quotations (RFQ) for a light-sheet microscope system. The National Institute of Neurological Disorders and Stroke (NINDS) requires one light-sheet microscope system to examine the circuits surrounding different subpopulations of dopamine neurons in the substantia nigra. The amendment provides responses to vendor questions regarding the RFQ, including clarification on where to submit formal responses, which RFQ attachments need to be completed and submitted, and whether documents can be submitted as a single consolidated file or as separate attachments. The RFQ number is 75N95024Q00465, and the amendment indicates that all other terms and conditions remain unchanged.

Question Number Question and Answer

1.

Question: Where should vendors submit their formal responses?

Answer: In accordance with the RFQ’s instructions, all offerors must email their quotations and any attachments directly to evan.feely@nih.gov to be considered for award.

2.

Question: Could you please clarify the nature of the various RFQ attachments and which ones need to be completed and submitted with a vendor’s formal response?

Answer:

Combined Synopsis/Solicitation (This is an overview of the RFQ, including the quotation submission instructions and the evaluation procedures. All required information (either included directly within quotation or on cover page) is listed on page 2.)

Attachment 1 – Purchase Description (This describes the minimum technical specifications and delivery requirements for the required equipment; this information should be reflected in the quotation).

Attachment 2 – Additional Terms and Conditions (These are Government terms that are incorporated into the RFQ and any resulting contract. An offeror’s quotation simply needs to include a statement that it acknowledges and accepts these terms unless specific concerns are identified.)

Attachment 3 – Representations and Certifications (These need to be completed and submitted with an offeror’s quotation. If an offeror’s online reps and certs completed at SAM.gov are current, it has previously represented that it does not provide prohibited telecommunications equipment or services, and all of its’ proposed equipment is domestically manufactured, it only needs to write “N/A” in the highlighted field on page 9, select “yes” or “no” for the highlighted provision on page 26, and sign at page 27).

Attachment 4 – IPP Invoicing Instructions (These are the instructions for submitting an invoice in the event an offeror is awarded a contract for this requirement. These instructions do not need to be addressed; this is simply a reference document).
