About this file

This document is an Addendum to 52.212-1 Instructions to Offerors for Commercial Products and Commercial Services, specifying quote submission requirements for a Pharmacist services contract. The solicitation requires offerors to submit four distinct volumes via email: (1) Administrative Documents including signed SF-1449 and representations, (2) Technical Capability and Approach demonstrating PWS requirements with key personnel details and qualifications, (3) Past Performance with at least three pharmacist service projects completed within the past five years, and (4) Pricing with a detailed breakdown of hourly rates including personnel costs, fringe, benefits, taxes, overhead, and profit.

Key submission requirements include identifying key personnel qualifications, providing a retention plan to prevent service interruptions, and submitting comprehensive pricing information. Quotes must be emailed to Melissa.Lake@ihs.gov, with pricing information strictly prohibited in Volumes 2 and 3. The government reserves the right to review additional past performance assessments beyond what is submitted and may exclude quotes that do not meet administrative or Terms and Conditions requirements.

NOTICE: The government reserves the right to consider contractor past performance assessments beyond what is submitted by the prospective contractor.

(4) Volume 4 – Label “Price.” Offerors shall provide detailed pricing aligned with the technical approach.

Include a narrative that supports the pricing and indicate any discount terms.
