Required information includes the name of the federally recognized tribal entity, the 51% owner's name, business name, UEI number, and certifying signature with printed name. False or misleading information submitted in connection with this certification constitutes a violation of federal law and may result in penalties under 18 U.S.C. 1001 for false statements or 31 U.S.C. 3729-3731 and 18 U.S.C. 287 for false contract performance claims.
