Background Information

The overall goal of the IPC Program is to transform the health care system by improving the quality of and access to care for American Indian and Alaska Native (AI/AN) people. The Patient Centered Medical Home (PCMH) model of care processes have been integrated into IPC improvement work as a framework for system wide quality improvement. The IHS plans to expand this work to assist all IHS ambulatory care facilities, including hospital based ambulatory care, health centers and health clinics to attain a PCMH designation in the Indian health system.

The ultimate goal is to provide the support needed by IHS ambulatory care facilities to achieve PCMH recognition and designation by 2021, and to benchmark levels of quality improvement capabilities, efficiency and patient experience.

The IPC program continues to assist care teams in the I/T/U to build capability in quality improvement science skills and knowledge to make changes and improvements in the way they organize and provide care.

The work of IPC supports fundamental changes in the system of care to build effective relationships for healthcare that is patient, family and community-centered. The IPC program is a structured, active learning community in which the participating organizations share results from changes tested. The impact of these changes is improved access and continuity of care, decreased utilization of emergent and urgent care, improved staff satisfaction, and improved health and health care outcomes as measured by the Clinical Reporting System (CRS) and iCare, and reported as Government Performance and Reporting Act (GPRA) measures and a core set of measures for the sites that participate in IPC. Health programs participating in IPC have shown measurable improvements in care and will contribute to a larger goal of eliminating health disparities that currently exist for AI/AN people.
