Quality Improvement is a priority of the IHS agency. Currently the IHS healthcare system lacks the capability in science and improvement knowledge and skills to provide technical assistance and training for ambulatory facility care teams. There is also a lack of subject matter expertise within IHS to lead training efforts for all IHS ambulatory facilities to meet PCMH requirements to achieve PCMH implementation and designation by 2021. There is a need to improve this capability in order to develop Subject Matter Experts to equip them with the necessary skills, knowledge, and capability to define, develop, test and implement quality improvement efforts for the agency using quality improvement science training. The Improving Patient Care Program (IPC) will identify 15 subject matter experts to be trained in the Healthcare Improvement Professional/Improvement Advisor course to then provide Quality Improvement Training and support to implement quality improvement efforts across the 12 IHS Areas.

The IPC program will also identify 15 individuals to participate in Refresher Course

Profound Knowledge Partners Inc. is the only service provider that can provide this particular type of HIP/IA training as it is a comprehensive course that was developed and trademarked through PKP Inc., with no other course content like it in the IA training field. This training offers the Improvement Advisor course content on an accelerated level which is different than the IA Training course offered through the Institute for Healthcare Improvement (IHI). PKP, Inc. has the capability to tailor the training to meet the quality improvement training needs for the agency. The PKP Inc. HIP/IA Training will focus on developing those participants with the capability to define, develop, test and implement improvement efforts using the science of improvement.

PKP Inc. can provide the necessary Improvement Advisor training for fifteen participants at a savings to the agency. Other IA course such as the Institute for Healthcare Improvement (IHI) offer training in Boston, MA.

The agency would have to pay for travel and per diem at a high cost for that region. The PKP Inc. IA course would convene at the IHS headquarters with necessary travel for only 6 Area participants; the remaining staff are already at the headquarters offices. (See below) As part of the overall training course, PKP Inc. has entitled their course the “Improvement Professionals” Workshop and have incorporated an Accelerated Model for Improvement known as the “Ami™ Workshop into the course. The PKP Inc. HIOP/IA training offers the standard content including the science of improvement and quality improvement methods, concepts, and models. PKP Inc. has incorporated an accelerated model for improvement that is different from the traditional Model for Improvement used in the IHI Improvement Advisor course.

Each of these training activities are trademarked and not found in other trainings. IHS participants will benefit from the accelerated learning models including eLearning courses, HIP wave portal, Active Ami™ Teams, Analysis and Development of Actionable Charters Using the Accelerated Model for Improvement (Ami™) Methodology, Development of HIP/IA Using Existing, Active & Future Teams, and Planned Experimentation using MidState interactive program, the HIP/IA will learn theory, design experiments, collect data and analyze the results. These activities are designed to use planned experimentation and Enumerative and Analytic Analysis not found in other trainings. The trademarked activities contained in the Ami™ model are designed for the participant to work at an accelerated level and to see progress in leading their Areas to greater increases in quality improvement process.

PKP Inc. will provide the necessary training to develop Improvement Professionals who will lead the IHS organization into systems level changes by executing high level improvement projects through knowledge of the Science of Improvement. Upon completion of the Improvement Professional course the participants are poised to become effective leaders in IHS and share best practices in quality improvement across the 12 IHS Areas.

HIP Training:

To provide the Improvement Professional Workshop to train Indian Health Service candidates as Healthcare Improvement Professionals also known as Improvement Advisors (IA).

Improvement Advisor Training for fifteen (15) staff of the IHS agency to develop Subject Matter Experts to equip them with the necessary skills, knowledge, and capability to define, develop, test and implement quality improvement efforts for the agency using the science of improvement.

6. Description of the efforts made to ensure that offers are solicited from as many potential sources as practicable. Indicate whether a FedBizOpps notice was or will be publicized as required by FAR Subpart 5.2 and, if not, which exception under FAR 5.202 applies.

7. Explain the efforts taken (or to be taken) to solicit as many potential offerors as possible, particularly if the basis of the JOFOC is the authority cited in FAR 6.302-2 (i.e., unusual and compelling urgency) or 6.302-6 (i.e., national security.) (NOTE: Proposed acquisitions for other than full and open that are expected to exceed $25,000 must be announced in Federal Business Opportunities (FedBizOpps), whether or not a sources sought notice or other presolicitation notice was issued previously.) 8. Determination by the Contracting Officer that the anticipated cost/price to the Government will be fair and reasonable.

The Institute for Healthcare Improvement (IHI) provides a training that has some of the features of the Improvement Professional workshop. The IHI course must be taken at dates and time prescribed by IHI and is located in Boston MA.

PKP Inc. provides more content regarding quality improvement science and its implementation with 4 days of training for 4 dates at a lower cost and is able to provide the training at IHS HQ based on IHS timeline.

9. Description of the market research conducted (see FAR Part 10) and the results, or a statement of the reasons market research was not conducted.

The market research was conducted October 2018 to inquire as to the costs, content, dates, and availability of providing the Improvement Advisor training for IPC and IHS staff through the Institute for Healthcare Improvement. IPC staff reviewed the IHI website. The information was used to determine the cost effectiveness of sending 15 or more staff to Boston, MA for the IA training.

Research was conducted into various Improvement Advisor trainings to determine what type of knowledge and IA training they provide and if it is on an accelerated level to develop Subject Matter experts in improvement science:

IHA – Illinois Health and Hospital Association IHA is dedicated to offering Improvement Advisor site visits to HIIN hospitals that request assistance. William Peters is an improvement sciences expert who specializes in quality, service and financial improvement whom IHA has partnered with to deliver a variety of Improvement Advisor site visits in the HIIN. William is an Institute for Healthcare Improvement trained "Improvement Advisor" and serves as IHI Faculty on both domestic and international projects.

HRSA – Quality Improvement and Risk Management Training This program teaches how to design and implement a successful Quality Improvement program that meets core program and FTCA requirements and drives quality-related activities. The QI Planning Learning Series offer three sessions but was last offered in March 2011.

Partnership HealthPlan of California’s Improvement Academy - “Developing Leaders to ADVANCE Quality Excellence” ADVANCE is a 12-month Improvement Advisor training program that prepares participants to lead and sustain health care quality improvement initiatives. It was last offered in March of 2018.

Most quality improvement training programs offered many of the same course topics including basic quality improvement, Model for Improvement (MFI), Run charts, team based care, and change management. No course that was reviewed offered an accelerated type course such as the AMI which is an accelerated model of the MFI.

Furthermore, this justification will be publicized on www.fbo.gov to seek other service providers for the future requirements.
