A. INTRODUCTION AND BACKGROUND:
The U.S. President's Emergency Plan for AIDS Relief (PEPFAR)--the U.S. Government initiative to help save the lives of those impacted by HIV around the world--is the largest commitment by any nation in history to combat a single disease internationally. Working in more than 50 countries, PEPFAR has saved and improved millions of lives, prevented millions of HIV infections, and changed the course of the global HIV/AIDS epidemic. PEPFAR is driven by a shared responsibility among donor and partner nations and others to make smart investments to save lives. Thanks in large part to U.S. leadership, tremendous progress has been made in the fight against global AIDS. Now in Phase Three: Focusing on Sustainable Control of the Epidemic, PEPFAR is focused on the ambitious Joint UN Programs for HIV/AIDS 90-90-90 global goals: 90 percent of people living with HIV know their status, 90 percent of people who know their status are accessing treatment, and 90 percent of people on treatment have suppressed viral loads by 2020. While the USG's PEPFAR Strategy for Accelerating HIV/AIDS Epidemic Control (2017-2020) focuses primarily on 13 high-burden countries, PEPFAR aims to support all countries with PEPFAR funding to reach epidemic control by targeting evidenced based interventions for populations at greatest risk of HIV in areas of greatest HIV incidence.
PEPFAR's program builds on past strengths and increases efficiencies by scaling up effective interventions and ensuring value.
PEPFAR Regionalization Approach – Asia
PEPFAR is undertaking a major shift in Asia this year. The Regional Operational Plan (ROP) 2019 is pivotal for Asia, as PEPFAR regionalizes the seven former Strategic and Technical Alignment for Results (STAR) operating units (OU) and one Foreign Assistance Operational Plan (F-Op) country across Asia and Central Asia into one unified, 11-country Asia Region, with the aim of increasing efficiency, consolidating expertise and technical assistance, and advancing and sustaining epidemic control, with special attention to key populations and drivers of the epidemic.
Under this model, agencies will maintain USG programmatic leadership within countries, and overall PEPFAR regional coordination will occur through an Asia Region PEPFAR Coordination Unit in Bangkok, consisting of the Asia Region PEPFAR Coordinator and two senior USAID and CDC agency representatives, who will draw upon technical expertise from across the region.
All 11 countries share the challenges of: (1) needing more effective strategies to reach key populations (KP), their partners, and their networks with effective strategies for prevention, testing, treatment, and retention, and (2) improving the use of data systems and dashboards to optimize program improvement. The region will facilitate technical exchange among countries sharing effective approaches to KP, such as pre-exposure prophylaxis (PrEP), index and self-testing, gender-affirming service delivery, differentiated service delivery models, and initiation of same-day antiretroviral treatment (ART), reducing stigma and discrimination, unique identifier codes, and improving data use.
Regionalization was launched in November 2018 in Washington, DC, where PEPFAR field-based and headquarters-based agency leadership came together to create a joint understanding of optimal staffing for the regional program, initiate discussions about how to operate as a regional program, and prioritize PEPFAR investments to where they can make the most impact and the most significant contribution.
B. MAJOR ROLES AND RESPONSIBILITIES:
BASIC FUNCTIONS:
Under the direction of the Deputy Chief of Mission or his/her designate in Thailand, the Asia Region PEPFAR Coordinator ("Regional Coordinator") provides leadership and coordination in implementing the PEPFAR Strategy for Accelerating HIV/AIDS Epidemic Control for Burma, Cambodia, India, Indonesia, Kazakhstan, Kyrgyz Republic, Laos, Nepal, Papua New Guinea, Tajikistan, and Thailand consistent with PEPFAR goals and resources. As such, the Regional Coordinator, working closely with Agency Senior Representatives, will act as an OGAC focal person for PEPFAR Asia Region-related matters. S/he coordinates the broad program management for the multi-faceted PEPFAR Asia program; coordinates implementing agencies in the development, implementation, and supports the monitoring of regional HIV prevention, treatment, and care plans; serves as the key facilitator of the Asia Region PEPFAR USG inter-agency team implementing activities in the areas of prevention, treatment, and care.
The Asia Region PEPFAR Coordinator's main area of oversight is the facilitation, planning, coordination, and reporting of program performance for the inter-agency field programs in 11 countries in Asia. As directed by the Deputy Chief of Mission or his designate in Thailand, the Asia Region PEPFAR Coordinator will coordinate across the region with the PEPFAR implementing agencies to ensure that funds are allocated strategically across agencies and technical areas; coordinated with inputs from the partner country governments, donors, and civil society consistent with U.S. Government laws and policies, especially legislation pertaining specifically to PEPFAR; and that the allocations support the national goals and plans of the partner governments. The Asia Region PEPFAR Coordinator will also work with PEPFAR implementing agencies to coordinate reporting systems for tracking funds accurately so that resources are correctly directed and deployed rapidly.
PEPFAR is a “whole of government" program, involving multiple U.S. Government (USG) agencies in a single, strategic response coordinated by the Department of State at the headquarters level and by the Asia Region PEPFAR Coordinator in the field. An open, transparent, and effective inter-agency process is essential for the success of PEPFAR's efforts in HIV prevention, care, and treatment. The Asia Region PEPFAR Coordinator is responsible for coordinating the development and monitoring of a cohesive inter-agency program by providing facilitation to the inter-agency PEPFAR team in Asia for PEPFAR, which may include policy, technical, managerial, and operational aspects. The Asia Region PEPFAR Coordinator collaborates with the U.S. Embassy, multilateral organizations, and the Office of the U.S. Global AIDS Coordinator to ensure achievement, not only of the primary HIV prevention, treatment, and care goals of PEPFAR, but also its public health sustainability and health diplomacy goals.
