About this file

This is an attachments document for an RFP issued by the Virginia Department of Behavioral Health and Developmental Services (DBHDS) for Public Assistance Cost Allocation Plan (PACAP) consulting services. The document contains seven required attachments that must be completed and submitted with proposals: Offeror Data Profile, Pricing/Fee Schedule, Small Business Subcontracting Plan, State Corporation Commission Form, Proprietary/Confidential Information Identification, Proposed Changes, and Certification of Compliance. The pricing schedule requires bidders to provide costs for an initial audit report, improvement report, procedure format outline, quarterly narrative claiming templates updates, and cost allocation plan consultants.

The RFP includes Virginia's goal that 42% of purchases be made from small businesses, with evaluation points awarded based on proposed small business participation. Bidders must either be certified as a small business through Virginia's Department of Small Business and Supplier Diversity (DSBSD) or submit detailed plans for subcontracting with DSBSD-certified small businesses. The pricing structure is based on a not-to-exceed total price broken down by specific deliverables and hourly rates. Bidders must disclose any litigation history from the past 10 years, bankruptcy filings from the past 3 years, and demonstrate capacity to handle multiple concurrent clients.
