Please use the information below to submit a quotation. Note: Offeror’s may use another format for submission of quotes. However, all quotes shall contain the minimum information required under Federal Acquisition Regulation (FAR) 52.212-1, Instructions to Offeror’s, Commercial Items (Nov 2021) and include the information listed below.

Service Schedule:

1.1. Bi-weekly (every-other week) document destruction schedule. Contractor will provide a regularly scheduled time every-other week for onsite document destruction services.

1.1.1. List proposed schedule of services

1.2. Material shall be destroyed at a level that would make it unreadable, indecipherable, and irrecoverable, to recognize or reconstruct the information.

1.2.1. List proposed method of document destruction to adhere to requirements:

1.3. Containers provided shall be at minimum 65-gallons and lockable.

a) List size of container(s) provided

b) Is/are container(s) lockable

c) Are locks and keys provided by Contractor for each container

1.4. No material shall be taken from the facility to be destroyed at another location.

a) What method will be used to ensure documents are destroyed at service site?

1.5. Document destruction equipment shall be truck-mounted and must have the ability for a FEMA employee to witness destruction of documents while at the FEMA location?

1.5.1. Is document destruction service safely viewable by FEMA monitor?

1.6. Materials must be destroyed at the time of arrival. No material shall be left in a transport vehicle (truck) overnight for destruction the following day.

1.6.1. Provide a statement how this will be accomplished.

1.7. Contractor will assign an Account Point of Contact to communicate directly with the Government Appointed official to coordinate scheduled pickups and discuss matters or concerns with the services to be provided.

1.7.1. Provide a point of contact name, phone number and email address

1.8. The Contractor shall not begin document destruction until a FEMA monitor is present to witness the destruction process.

1.8.1. Provide a statement how this will be addressed if no FEMA monitor is available at the time of service.

1.9. The Contractor shall provide to the FEMA monitor a Certificate of Document Destruction for each shred occurrence for each shred bin.

1.9.1. Provide a statement how Certificates of Document Destruction will be delivered.
