About this file

This is a Past Performance Questionnaire template used to evaluate contractor performance on completed or ongoing federal contracts. The form captures comprehensive contract details including contractor name, address, contract number, procuring agency information, period of performance, and dollar values (award amount, annual amount, and total value with modifications). Contract type options include Firm Fixed Price, Time & Materials/Labor Hour, Cost Reimbursement, or other arrangements.

The questionnaire requires evaluators to assess contractor performance across seven key factors: Quality of Work, Personnel, Cost/Budget Control, Customer Satisfaction, Timeliness of Performance/Deliverables, Business Relations, and Overall Performance. Each factor must be rated using standardized adjectival ratings—Unsatisfactory, Marginal, Satisfactory, Very Good, or Exceptional—with mandatory supporting comments required for each evaluation area. The form concludes with a yes/no/maybe question regarding contractor selection for future work, signed by the evaluator with name, title, and date. Detailed rating guidelines are provided on page 3, specifying performance criteria and descriptors for each rating level across all evaluation categories, enabling consistent and objective assessment of contractor performance on federal contracts.
