About this file
This is a Past Performance Questionnaire template for FEMA's Individual Identity Verification and Authentication Support Services solicitation (70FB70-24-R-00000002). The questionnaire must be submitted by references to Contract Specialist Edward Ruud and Contracting Officer Danyelle Wilcox by December 11, 2024 at 11:00 EST.
The questionnaire evaluates contractors on four key performance elements: quality of services, customer satisfaction, timeliness of performance/contract completion, and ability to operate within cost estimates. References must rate performance as Superior, Satisfactory, Unsatisfactory, or Neutral for each element and provide supplemental comments. The form includes sections for the contractor's information, evaluator certification, and asks whether the reference would do business with the contractor again. All responses are kept confidential and will be used to evaluate offerors for the IDIQ contract for identity verification and authentication support services for FEMA disaster assistance programs.
