About this file
This is a Respondent Questionnaire attachment for Request for Quote (RFQ) No. 549635 issued by the City of Detroit Office of Contracting and Procurement (OCP) on behalf of the Department of Innovation and Technology (DoIT) to procure cyber security licensing services, specifically for CrowdStrike solutions. The questionnaire requires respondents to provide comprehensive information across two main sections: respondent experience and organization details, and respondent capacity and staffing capabilities. Respondents must identify a single point of contact, provide complete firm background information including legal business name, business entity designation, Federal Employer Identification Number, years in business, and employee counts for the past three years. Additionally, respondents must document at least three relevant projects completed within the last five years that demonstrate similar scope and size, including client references with contact information and dates of service.
The questionnaire also addresses Detroit workforce participation by requiring respondents to indicate the number of Detroit residents who would work on the project and projected new hires, with an option to sign a Priority Hire Agreement with Detroit at Work. Respondents must disclose any claims, lawsuits, contract terminations, debarments, or suspensions involving their organization or proposed subcontractors within specified timeframes and must identify whether their proposed solution involves artificial intelligence or will within five years. For any subcontractors, respondents must provide detailed information including legal business names, addresses, scope of work, percentage of work allocation, and prior project experience completed jointly with the prime contractor. Finally, respondents must submit organizational charts, resumes for key personnel, employee dedication levels, and relevant experience documentation to demonstrate capacity to perform the requested services.
