About this file
This document is a Conflict of Interest Questionnaire and Municipal Officials List from the City of Lubbock, Texas, required under Local Government Code Section 176. The file identifies eight municipal officials including Mayor Mark McBrayer and six council members representing districts 1-6, along with City Manager W. Jarrett Atkinson. The document provides the standard Texas Ethics Commission Form CIQ, which vendors must complete and submit to the City's records administrator within seven business days after becoming aware of facts requiring disclosure or after beginning contract discussions with the municipality. The questionnaire applies to vendors with business relationships with the City of Lubbock that involve employment or business relationships with local government officers exceeding $2,500 in taxable income over a 12-month period, gifts with aggregate values exceeding $100, or family relationships with city officials.
The form requires vendors to disclose any employment or business relationships with city officers or their family members, any taxable income received from such relationships, any gifts provided to officers or their families, and any business relationships maintained with entities where officers serve as directors or hold ownership interests of one percent or more. The questionnaire reflects amendments made by H.B. 23 of the 84th Legislative Session and applies specifically to vendors bidding on the City's Consulting and Broker Services for City Property and Casualty Insurance contract, which seeks professional insurance consultant/broker services for managing the City's insured and self-funded programs with coverage to begin April 1, 2026.
