Attachment J-2 Past Performance Questionnaire

Instructions to Offerors

All fields in Section I of each past performance questionnaire must be completed by the Offeror prior to issuing the questionnaires to the customer points of contact. The Offeror must include in their proposal submissions a copy of each questionnaire with Section I completed. (Section I of the questionnaires do not count toward page limits). In addition, the Offeror must send these questionnaires with Section I completed to the customer points of contact.

The customer points of contact are responsible for completing Section II of the questionnaire. For Government contracts, Section II of the form must be completed by the Contracting Officer (CO) or the CO’s Representative (COR). For commercial contracts, Section II of the form must be completed by personnel with duties similar to those of a CO or COR. It is the responsibility of the Offeror to ensure that the customer points of contact complete the questionnaires and submit them to the FAA in a timely manner. The completed questionnaires must be submitted directly from the customer point of contact by the proposal date to Kelly Chappelle, NEXTOR III Contracting Officer, via e-mail at Kelly.Chappelle@faa.gov.

Instructions to Customer Points of Contact (POCs)

In order to evaluate potential Offerors for the FAA’s Next Generation Academic Consortium for Aviation Operations Research (NEXTOR III) competition, the FAA contracting office is obtaining past performance references for each Offeror. As a reference for work performed by the Offeror and identified in Section I of this questionnaire, your candid responses in Section II will assist in the evaluation process. Please complete Section II of the questionnaire as thoroughly as possible, using the Contractor Rating Performance Criteria provided in Section III. Please rate the contractor support you have received from the Offeror within the past five (5) years on the contract identified in Section I of this questionnaire. If the contractor support was as a Prime Contractor, provide information and an evaluation for the entire contract. If the contractor support was as a Subcontractor, provide information and an evaluation only for the Offeror’s subcontract performance.

The completed Past Performance Questionnaire must be received by Kelly Chappelle, FAA Contracting Officer, via email at Kelly.Chappelle@faa.gov no later than the proposal due date. Please indicate “Past Performance Questionnaire Submission for <Offeror Name>” in the subject line of the submission email.

In addition to this questionnaire, you may receive a follow-up phone call or email to confirm or clarify information. If you have questions regarding this questionnaire or require assistance, please submit inquiries to Kelly Chappelle, NEXTOR III Contracting Officer, via email at Kelly.Chappelle@faa.gov.
