About this file

This is a Past Performance Questionnaire for federal contractors responding to Federal Aviation Administration (FAA) solicitation 693KA8-26-R-00004 for Online Mathematics Tutoring Services. The questionnaire is designed to assess contractor performance on similar past contracts and must be completed by the contracting officer or government representative with direct knowledge of the contractor's work. Completed responses must be submitted to Timothy Thannisch (AAQ-310) at the FAA, 800 Independence Avenue S.W., Washington DC 20591, or via email at timothy.g.thannisch@faa.gov no later than May 4th, 2026 at 12:00 PM Eastern Time.

The questionnaire uses a five-point rating scale (0=Poor, 1=Marginal, 2=Satisfactory, 3=Good, 4=Excellent) to evaluate contractor performance across two primary categories: Program Management and Tutoring Services. Program Management assessment covers personnel qualifications, task management, problem identification and resolution, quality control effectiveness, report timeliness, key personnel competency and communication, corporate support, contract terminations or reschedules, cure notices, and overall management performance. Tutoring Services evaluation focuses on the effectiveness of online mathematics tutoring delivery, 24/7 tutor availability, multi-device and browser compatibility (iOS, Android, Internet Explorer, Chrome), learner document upload capabilities, tutor subject matter expertise and knowledge transfer, monthly usage and progress reporting, Statement of Work relevance, and overall technical performance. Respondents must provide narrative explanations for any ratings of Marginal, Poor, or affirmative "Yes" responses, and may recommend the contractor for award of similar contracts based on demonstrated performance.
