Past Performance Survey Form

Instructions to Offerors Sending Reference Survey Forms:

All parts of Section A of this attachment must be completed by the Offeror (the firm requesting the reference) prior to issuing the survey. Offerors must include a copy of each requested survey as Volume III, Part A of its proposal.

The Offeror must send the Past Performance Survey in accordance with instructions in SIR Section L.5.3.

The Past Performance Survey must be provided to a customer that represents the Program Office or Contracts organization , i.e. Program Manager, Task Order Lead, Contracting Officer’s Representative, Contracting Officer, Contract Specialist, etc. For commercial references, send to personnel with duties similar to those for Government contracts. It is the responsibility of the Offeror to follow-up and encourage references to submit the survey by the due date of the proposal. The completed survey must be submitted to the FAA Contracting Officer by the proposal due date via e-mail directly from the person providing the reference.

Instructions to Person Providing Reference:

In order to assess potential vendors under the Dedicated Network Telecommunications System (DNTS) contract, the FAA contracting office is obtaining past performance information with respect to each Offeror.

As a reference for work performed by the firm identified in Section A of this survey, your candid response will assist in the evaluation process.

Please complete all parts of Section B of the survey as thoroughly as possible. The surveys must be submitted to Austin.W.Francis@faa.gov by January 31, 2024. The subject line in the submission e-mail must clearly indicate: SIR 693KA8-24-R-00007 – Submission of Past Performance Survey [Insert Offeror Name].

The message must originate from the reference’s government or commercial e-mail system.

In addition to this survey, you may receive a follow-up phone call to confirm or clarify information. This is a major FAA acquisition and your input into this evaluation is greatly appreciated.
