About this file
This document is a Past Performance Questionnaire issued by the Maritime Administration (MARAD) to solicit reference information regarding a contractor's past performance on vessel inspection services.
The questionnaire requests evaluations across four primary performance areas: (1) Quality of Product or Service, assessed through conformance to contract requirements, personnel appropriateness, report accuracy, and technical excellence; (2) Timeliness or Scheduling of Supplies/Deliveries, evaluated based on performance timeliness, interim milestone achievement, reliability, and responsiveness to technical and contractual direction; (3) Business Relations/Customer Satisfaction, measured through effective management, prompt problem notification, cooperative behavior, subcontract management, and small business subcontracting program effectiveness; and (4) Key Personnel and Staffing (Including Subcontractors), evaluated on personnel qualifications and effective performance and conduct. The questionnaire also includes an optional Cost Control section for cost contracts only. References must provide ratings using a five-point scale: Exceptional, Very Good, Satisfactory, Marginal, or Unsatisfactory, with detailed definitions provided for each rating level. Additionally, references must indicate whether they would hire the contractor for future services. Completed questionnaires must be submitted to Henry Puppe, Contracting Officer, at Henry.Puppe@dot.gov by May 1, 2020, the solicitation closing date. The questionnaire includes fields for contractor name, contract number, evaluation period, and delivery order number.
