The NAICS code for this action is 524210, and the procurement is managed by the Department of Transportation Maritime Administration (DOT/MARAD). The U.S. Merchant Marine Academy (USMMA) is interested in utilizing a PPO (Preferred Provider Organization) approach to offer a Student Health Insurance Policy (SHIP) that will provide healthcare insurance coverage to the general population of USMMA students (hereby referred to as Midshipmen/Plebes/Plebe Candidates). Coverage will need to encompass on or off campus care 24 hours a day, 7 days a week, worldwide. This requirement will be a zero-dollar contract and will be paid by the Midshipmen directly to the broker. This Request for Information to find qualified, licensed and experienced in Marine insurance, sources who could provide above support.
All interested vendors are requested to provide their capability statements and respond to the following questions:
1. .Provide complete name, address, POC name and phone and email, indicate large or small business under which NAICS code to meet this need.
2. Do you provide a hard-wavier program, requiring Students to purchase a Student health insurance plan with the ability to waive purchase of that plan (providing the insurance plan meets waiver requirements as stipulated by USMMA and meets all the SHIP insurance coverage requirements)?
3. Describe your ability to provide internet-based purchase of health insurance vs. waiving said purchase if a private health insurance plan exists that meets required coverage needs. Is this website flexible to accommodate special enrollment needs, such as enrolling/ waiving insurance coverage for 6-month vs 12 months with 50% reductions in policy costs? Will Students be capable of viewing policies on line, downloading insurance I.D. cards?
4. Describe your ability to provide coverage for but not limited to preventive care, doctor visits, emergency care, non-emergency care, prescription medications, mental health services, health screenings/ immunizations, maternity/reproductive care, hospitalization/surgery and offer both in-network and out-of-network options for care.

6. Are you capable of providing access to both in network & out of network providers in the New York/Long Island area who practice within 10 Miles of the USMMA (300 Steamboat Road, Kings Point, NY 11024) within the Nassau County area?
7. Can you provide a deferred payment system where a Midshipman/Student waiting for financial aid awards would be insured and able to suspend payment in lieu of receipt of financial aid dollars when received to be used to satisfy any deferred payment?
8. USMMA requires Midshipmen/Students to be responsible for the payments of the policy premiums directly to the Carrier. USMMA will not be a party to this reference contract between the Midshipmen and the insurance company. The Carrier will be responsible for obtaining and tracking all payments. Are you capable of meeting this requirement?
9. Can you provide access to telephonic and/or virtual real time health care / behavioral health support providing reliable health information in response to health concerns and questions and assistance in decisions on the appropriate level of care for an injury or sickness, 24/7/365 days, globally and within the United States.
