About this file
This is a Statement of Work (SOW) for a water service line installation project located at 9420 Celia Berryhill Rd., Okmulgee, OK 74447. The project requires furnishing and installing a 2-inch ultra-high molecular weight polyethylene (PE) water service line with specific technical requirements, including non-flare compression fittings, a brass ball valve, and a polyethylene valve box. The installation must follow detailed specifications for trench depth (minimum 30 inches), backfilling, pipe placement, and utility crossings, with strict requirements for pipe support, compaction, and site restoration.
The project is part of a federal solicitation (RFQ1514595) issued by the Indian Health Service, with a 60-calendar day performance period and an estimated award value under $25,000. It is a 100% Indian Owned set-aside under NAICS code 237110. Key dates include a July 3, 2025 deadline for questions and a July 10, 2025 quote submission deadline. Contractors must provide a firm fixed price, technical specifications, two relevant past performance references, and complete the Indian Economic Enterprise Representation Form. Quotes will be evaluated on an acceptable/non-acceptable basis, with ranking proceeding from lowest to highest price.
