About this file
This is a Proposer Information and Certification Sheet for Request for Proposal #6223, issued by the State of Oregon's Alcohol and Drug Policy Commission through the Department of Administrative Services (DAS) Procurement Services. The RFP seeks a contractor to update and develop new analyses and reporting on Oregon's Substance Use Disorder Continuum of Care through an iterative and collaborative process with the ADPC team. The contractor will design, develop, and deliver analyses to enhance the Commission's understanding of services delivered through the SUD continuum of care. Proposals constitute firm offers for 180 days following the closing date, and if awarded, the contractor agrees to perform the scope of work and meet performance standards set forth in the final negotiated scope of work.
The certification sheet requires proposers to provide legal entity information, state of incorporation, contact details, and Oregon Business Registry Number if applicable. Proposers must certify compliance with multiple statutory requirements, including tax law compliance, non-discrimination in employment and subcontracting practices, wage equality standards under ORS 652.220, and broadband Internet access service regulations if applicable. Additionally, proposers must certify that they are not listed on the Office of Foreign Assets Control's Specially Designated Nationals list or the System for Award Management exclusions lists, have no conflicts of interest with the State, and that all proposal contents are truthful and prepared independently without collusion. Proposers with anticipated contract prices of $150,000 or more must certify compliance with ORS 279A.112 regarding written policies and practices to prevent sexual harassment, sexual assault, and discrimination. False or fraudulent statements in response to the RFP may constitute false claims under the Oregon False Claims Act.
