About this file
This is a sworn statement form required pursuant to Kentucky Revised Statute (KRS) 45A.395 for the City of Owensboro, Kentucky's Purchasing Department. The document serves as a prerequisite form that any bidder or offeror must submit as part of their bid submission to be considered a responsible bidder. The form is dated for completion in 2025 and includes spaces for the bidder's company name, authorized representative signature, and title, though no specific procurement details, response dates, or contract terms are provided in this compliance document.
The statement requires bidders to swear under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated Kentucky's campaign finance laws, and that awarding a contract to them would not violate these laws. The document defines "knowingly" as being aware or should have been aware of conduct or circumstances that constitute an offense. No pricing information, set-asides, funding sources, or incumbent details are included, as this is solely a legal compliance certification form rather than a solicitation document. The City of Owensboro's Purchasing Department contact information is provided, including their address at 101 E. 4th Street, P.O. Box 10003, Owensboro, KY 42302-9003, phone number 270-687-8431, and website www.owensboro.org.
