About this file

This is a Pricing and Delivery Schedule form from the University of Texas Health Science Center at San Antonio (UTHSA) Department of Purchasing for RFP 745-26-P11, which concerns the procurement of Laptop Protection Services for students in the School of Dentistry. The solicitation seeks proposals from qualified vendors to provide a comprehensive secondary laptop insurance protection plan designed to supplement manufacturer warranties and existing protection programs by covering accidental damage, loss, and theft of student-owned laptops. The contract term consists of an initial period of up to three years, with the option for the University to renew the agreement for up to three additional one-year terms, subject to the same terms and conditions. The RFP does not guarantee any specific volume, minimum, or maximum compensation for services provided.

Proposers must submit detailed rate sheets identifying all hourly rates and titles of team members associated with the account, with hourly rates remaining firm throughout the initial term and any renewal periods. All pricing must include labor, equipment, materials, fees, overhead, profit, insurance, licenses, and all other related costs necessary to fulfill the scope of work. The University reimburses reasonable and pre-approved expenses, including meals, ground transportation, coach-class airfare, and lodging, in accordance with current Texas Comptroller of Public Accounts travel reimbursement rates, subject to a mutually agreed maximum expense cap. Payment terms are net thirty days in compliance with the Texas Prompt Payment Act, with the University accepting electronic funds transfer methods including ACH and single-use account cards. Proposers may offer prompt payment discounts and must submit invoices electronically to AP-einvoice@uthscsa.edu with valid purchase order references. The University is exempt from Texas sales and use tax as a state agency.
