About this file
This is a proprietary information, qualification questionnaire, and mandatory requirements document for the City of Fort Worth Purchasing Division covering multiple Request for Quotes (RFQs). The document contains inconsistent reference numbers across pages, with titles including "Grading Services" (RFQt 25-0228), "Scantron" (RFQt 25-0227), and "Pipe Plug" (RFQt 25-0205), suggesting this is a template form used across different procurement opportunities. The questionnaire evaluates vendor qualifications through questions about business operations, experience, email responsiveness, sales locations within Fort Worth city limits, and service capabilities. Required submission documents include completed forms RFQt-02 (Form CIQ), RFQt-03 (Authorized Signed and Requirements), RFQt-06 (Questionnaire and Requirements), signed addendum acknowledgments, and bid tables.
The document addresses cooperative purchasing agreements availability, prompt payment discount terms with percentage and day specifications, additional services pricing as catalog discounts, and vendor service agreement modifications. Vendors must identify any proprietary information by marking pages with "PROPRIETARY" in yellow highlighting, though pricing information cannot be marked proprietary and will be publicly disclosed in bid tabulations. The form emphasizes that all submitted materials become public property under the Texas Public Information Act, with final disclosure decisions resting with the Texas Attorney General.

i. All material submitted to the City becomes public property and is subject to the Texas Public Information Act upon receipt.
ii. If a Bidder does not desire information that it believes to be proprietary in the bid to be disclosed, the bidder must list each page number in the chart below and must clearly mark and identify each page by including the word “PROPRIETARY” in all caps and highlighted yellow on the bottom center of each page at the time of submittal. The City will, to the extent allowed by law, endeavor to protect such information from disclosure. The final decision as to the information that must be disclosed lies with the Texas Attorney General. Note: Pricing may not be marked proprietary and will be made publicly available in the form of a bid tab following evaluation of all bids. By submitting a response, Bidders agree that pricing is not proprietary and may be released.
