About this file
This document is a Required Form for M/W/SBE Utilization from the City of Charlotte's Business Inclusion program, focused on promoting diversity in city contracting and procurement processes. The form requires companies responding to a Request for Proposal (RFP) to outline their plans for including City-certified Small Business Enterprises (SBE), Minority-owned Business Enterprises (MBE), and Woman-owned Business Enterprises (WBE) in their proposal and contract execution.
The form requires bidders to indicate their own MWSBE status, provide details of any current certifications, and describe both past and planned outreach efforts to maximize inclusion of minority, women, and small businesses. Bidders must complete a table listing potential MWSBE subcontractors, including their names, work descriptions, business classification (M/W/S), and city vendor numbers. The form also includes fields for calculating total utilization percentages for MBE, WBE, SBE, and combined MWSBE participation, demonstrating the city's commitment to supporting diverse business participation in municipal contracts.
