About this file

This is a Schedule B Pricing document for Request for Proposal No. RFP 260000001876 issued for Child Nutrition Program Monitoring and Compliance Reviews. The document establishes the pricing submission requirements for contractors responding to this state procurement opportunity. Contractors must submit a completed pricing schedule using the provided Schedule B Pricing Matrix in a modifiable format such as Microsoft Word or Excel, though an additional non-modifiable PDF version may be submitted as a supplement. Failure to complete the pricing schedule as requested may result in disqualification of the proposal.

Price proposals must be comprehensive and include all costs associated with contract performance, encompassing one-time or set-up charges, fees, shipping and handling, per-piece pricing, palletizing, and any other potential costs that the Contractor may charge the State. Contractors are encouraged to offer quick payment discount terms, specifying a percentage discount off the invoice if payment is received within a stated number of days following invoice receipt, with the requirement that processing time for payment receipt by the Contractor's financial institution not be included in the day count. By submitting a proposal, the Contractor certifies that all prices were developed independently and without consultation, communication, or agreement with any other Contractor.
