About this file

This document is a Non-Conflict of Interest certification form required by the Colorado Department of Corrections for offerors participating in the YOS-Residential Community Transition Services procurement process. The form requires authorized signers to certify compliance with Colorado Revised Statutes sections 18-8-302, 18-8-306, 18-8-301, and 18-8-308, as well as the State of Colorado State Controller Policy on Procurement Conflicts of Interest. Signers must affirm they have no actual, potential, perceived, or apparent conflicts of interest and acknowledge their access to State information and records during the procurement process.

The certification establishes that the signatory understands the Procurement Official's authority to remove participants from the process as deemed appropriate and that participants may request removal at any time. Signers must declare they have no personal or beneficial interest in the services or property described in the relevant statutes and commit to immediately notifying the Procurement Agent of any situation that could alter previous representations or create an appearance of conflict or impropriety. The form includes the signatory's acknowledgment that they are an employee of a private employer or independent contractor under contract with the State of Colorado and are receiving procurement information in that official capacity.
