About this file
This is a Vendor Qualifications Certification attachment for an Independent Verification and Validation (IV&V) Services contract issued by the Illinois Department of Human Services (HFS) for the Child Support Automation and Innovation for Modernization project (Contract ID: 2026-55-119/26-478HFS-MEDPR-B-51464). The certification document establishes mandatory responsiveness requirements that vendors must satisfy to be considered for award. Vendors must demonstrate continuous operation providing IV&V services for child support programs for a minimum of five years immediately preceding the bid opening date, supported by current business registration documentation. Additionally, vendors must provide evidence of at least three qualifying large-scale business process and systems implementation planning and installation projects completed within the past ten years, with at least one project demonstrating direct child support program experience.
The vendor qualification requirements include demonstrated experience working directly with the Office of Child Support Enforcement (OCSE) in conjunction with IV&V activities and expertise with federal planning documents and certifications necessary for child support program compliance. All qualifying projects must have been performed by the bidding vendor entity as the prime contractor, with no experience based solely on individual employee work history from previous employers or subcontractor relationships, unless the vendor is a legally continuous predecessor entity. Vendors must certify under penalty of law that all submitted information is true, accurate, and complete, and acknowledge that failure to meet any requirement or submission of false information will render the bid non-responsive and may subject the vendor to applicable legal remedies. The certification requires signature by an authorized company signatory with title and date.
