About this file

This is a Respondent Questionnaire attachment for a Request for Quotation (RFQ) issued by the City of Detroit Office of Contracting and Procurement (OCP) on behalf of the Home Revitalization Department's Bridging Neighborhoods Division. The RFQ seeks qualified Environmental Contractors to provide Hazardous Material Abatement services for homes selected for renovation and demolition within the Bridging Neighborhoods program. The contract will be awarded for a 2-year term, during which the Bridging Neighborhoods Division estimates it will renovate approximately 5-7 homes. The questionnaire requires respondents to provide detailed contact information for a single point of contact and project manager, complete background information about their firm including business entity designation, tax status, Federal Employer Identification Number, years in business, and employee headcount, and disclose any prior experience with the City of Detroit.

Respondents must document at least three relevant projects completed within the last three years that demonstrate their ability to manage contracts of similar size and scope, including detailed reference information and key personnel assignments. The questionnaire requires disclosure of any claims, lawsuits, contract terminations, or debarment or suspension status for the respondent and any proposed subcontractors or suppliers. If subcontractors will be utilized, respondents must provide their legal business names, addresses, descriptions of services, percentage of work allocation, and at least one reference demonstrating relevant experience completed within the last five years. Additionally, respondents must submit an organizational chart identifying key personnel and resumes for all key personnel meeting the minimum qualifications specified in the RFQ.
