About this file
This document is an Asbestos Inspection Report for the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio, prepared by Sierra Environmental Group, Inc. in March 2011. The report details a comprehensive asbestos hazard evaluation conducted by certified asbestos hazard evaluation specialists Chuck Callahan and Todd Taylor. The inspection involved collecting and analyzing 54 bulk samples from 32 homogeneous areas to identify asbestos-containing building materials (ACBM). The report found one sample containing over 1% asbestos, specifically a preformed-block pipe insulation sample with 20% chrysotile asbestos.
The report recommends an Operations and Maintenance (O&M) Program to manage the ACBM, including appointing an Asbestos Program Manager to oversee maintenance and renovation activities. The preformed-block pipe insulation, located in poor condition, requires immediate response action, including potential evacuation, removal, enclosure, or encapsulation. The exterior stucco and roofing materials, which are nonfriable and in good condition, do not require immediate action. The total estimated removal costs for the identified ACBM vary, with the preformed-block pipe insulation estimated at $350, exterior stucco at $80,000, and roofing materials at $46,950. The report also emphasizes the need to comply with OSHA and AHERA regulations regarding asbestos management, notification, and potential removal procedures.

SECTION 1
EXECUTIVE SUMMARY
A total of fifty-four bulk samples were collected from the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio and analyzed for asbestos fiber content by Polarized Light Microscopy. One of the samples was found to contain> I% asbestos.
The fifty-four bulk samples were collected from thirty-two homogeneous areas (a Homogeneous Area is defined as "an area of surfacing material, thermal system insulation, or miscellaneous material that is uniform in color and texture"). There was one identified homogeneous area in the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio that were sampled and determined to be asbestos-containing building materials (ACBM). There are two homogeneous areas in Dunham Allen House that are assumed to be asbestos-containing building materials (ACBM). These materials are as follows:
Preformed-block pipe insulation (10 If) Exterior stucco (16,000 sf) Roofing materials, assumed (9,390 sf)
Firedoors, gaskets, electrical wire sheathing, firebrick, roofing, boiler muds, etc., which were not sampled, should be assumed to contain asbestos. All activities involving these materials should be in strict accordance to requirements stipulated by the EPA, unless further testing proves these materials to be asbestos free. Asbestos-containing materials, which may be located behind walls, above hard plaster ceilings, under carpeting, etc., were not assessed or quantified. If, through demolition or renovation, these materials are discovered, they should be documented and handled accordingly.

SECTION 2
INTRODUCTION
In September 20 I0, Hull & Associates contracted Sierra Environmental Group, Inc. (Sierra) to conduct a comprehensive building inspection of the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio for asbestos-containing building materials (ACBM).
The purpose of the inspection program was I) to identify those accessible building materials that may contain asbestos, 2) to collect and analyze bulk samples in accordance with EPA-recommended guidelines, 3) to physically assess the condition of all accessible asbestos-containing material (ACM) and its potential for disturbance, and 4) to provide estimated 20 II asbestos unit removal costs. Utilizing the assessment of ACBM, Sierra will recommend appropriate strategies to control or eliminate potential hazards.
Mr. Todd Taylor and Mr. Chuck Callahan, (Ohio-Certified Asbestos Hazard Evaluation Specialist # ES32139 and # ES31960) with Sierra, conducted a detailed walk-through inspection of the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio from March 16-17,2011. Representative bulk samples of suspect materials were collected and condition of the materials were assessed.
Section I of this report is the Executive Summary that briefly discusses the ACBM that was identified from the laboratory analysis of suspect materials, and the material that was assumed to be ACBM.
Sections 3 through 6 consist of the Building Inspection Report, which describes the building inspection procedures, bulk sample collection and analytical techniques, and the material evaluation and physical assessment methods used.
It also explains how estimates were made of the footages of areas treated or covered with ACBM, as well as a listing of types of material tested, their location, and their asbestos content.
In order to obtain specific knowledge of the ACBM in the building, utilization of Tables 3-1 and 6-1 is essential.
Table 3-1 lists all of the materials which were sampled, sample locations, sample numbers (field and laboratory), asbestos content, and homogeneous area numbers. A description of all homogeneous area numbers can be found at the end of Table 3-1. Table 6-1 lists all of the areas in the building where ACBM was identified. By using Table 6- I and the homogeneous area numbers found at the end of it, one can determine how much of each variety of ACBM exists in a particular area. Type ACM, condition, potential for disturbance, response action cost estimates and 20 I I unit removal cost estimates are also summarized on this Table.
Cost estimates for asbestos-containing materials are calculated based on 20 II unit rates. A cost estimate has been provided for each independent line item, with a minimum estimate of $250.00. Because costs were estimated for individual items, the actual cost to remove several line items would be significantly lower than the aggregate of their individual cost estimates.
Proof of the Sierra inspector's training and Ohio Department of Health Certifications are presented in Appendix A.
Appendix B is a Glossary of terms that are commonly used throughout the Inspection Report and the asbestos industry in general.

BUILDING INSPECTION REPORT
SECTION 3
INSPECTION PROCEDURES
The asbestos inspection of the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio was scheduled through Mr. Matt Mullin, City of Cincinnati. The building inspection was conducted from March 16-17, 20 II by Mr. Todd Taylor and Mr. Chuck Callahan, (Ohio-Certified Asbestos Hazard Evaluation Specialists # ES32139 and # ES31960).
Upon arrival at the building the inspectors conducted a cursory review of available floor plans to identify those building components that could potentially contain asbestos. This information helped to familiarize Sierra with the building itself and with the locations of suspect building components. Regardless of the information gathered before the inspection, the renovation areas were given a thorough inspection. This inspection included a room-by-room walk-through investigation of the Dunham Allen House located at 4356 Dunham Way in Cincinnati, Ohio, during which the accessible areas were visually checked for the suspected presence of any asbestos-containing building materials (ACBM).
During the building inspection, the inspectors looked for suspect materials on building components such as pipes, tanks, walls, ceilings, etc. that could contain asbestos. Suspect materials were touched to determine their degree of friability, and sampling areas containing homogeneous materials were identified.
3.1 METHOD OF SAMPLING AND ANALYSIS
3.1.1 Bulk Sample Collection Method
During the building inspection, the asbestos evaluation specialist looked for suspect materials on building components such as pipes, tanks, wall, ceilings, etc. that could contain asbestos and pose potential exposure hazards.
Samples of suspect materials were collected in accordance with protocol outlined in the Asbestos Hazard Emergency Response Act (AHERA - Section 763.86). Although the AHERA regulation currently applies only to schools, it is considered state of the art in terms of protocol for building inspections for ACBM.
For friable Surfacing Materials, three (3) bulk samples were collected for each homogeneous area less than or equal to 1,000 square feet. If the homogeneous area was greater than 1,000 square feet but less than 5,000 square feet, five
(5) bulk samples were collected. For homogeneous areas that were greater than 5,000 square feet, seven (7) bulk samples were collected. The inspectors determined random sample locations in each homogeneous area of suspect surfacing materials during the initial inspection.
Three (3) samples were randomly collected from homogeneous areas of Thermal System Insulation such as piping.
Miscellaneous Material and non friable suspected ACBM were typically sampled "in a manner sufficient to determine" whether the material in question contained asbestos.
To avoid disturbing the material more than necessary and potentially cause the release of asbestos fibers, the inspectors performed bulk sampling of suspect materials in accordance with generally accepted procedures outlined in the current EPA Guidance Document and in accordance with AHERA (Section 763.86) protocol. Each sample was collected and placed in a clean, sealable vial and labeled with a unique sample identification number. This sample number was recorded on a Bulk Sample Log Sheet, the sample vial, and at the sample location to permit easy identification of the sampled materials in the future. Supplemental information was also recorded on the Bulk Sample Log Sheet, including date of inspection, name of the inspectors, the building name (or number), a brief description and location of the sample, and type of material sampled (e.g. thermal insulation, fireproofing, acoustical plaster). BuLk samples were submitted to IATL Laboratory or Environmental Hazards Services (EHS) analytical laboratory upon completion of the inspection.
3-1
3.1.2 Analysis of Bulk Samples
Bulk samples were analyzed for asbestos content by polarized-light microscopy (PLM) and dispersion staining (Method Reference: 40 CFR Part 763, Volume 47, No. 103, May 27, 1982, pg. 23376). This analytical method, which the EPA currently recommends for the determination of asbestos in bulk samples of friable insulation materials, can be used for qualitative identification of six morphologically different types of asbestos fibers:
chrysotile, amosite, crocidolite, anthophyllite, tremolite, and actinolite asbestos.
The method specifies that the asbestos content in a bulk sample shall be estimated and reported as a finite percentage (rounded to the nearest percent) within the range of 0 to 100. Minute quantities of asbestos in bulk samples may be reported as "trace" or less than I percent «1%). The analytical method determined the "area percent" asbestos or the percentage of the area ofa microscopic field of view that is occupied by asbestos fibers.
The results of all bulk sample analyses are reported in a standard written laboratory report. This written report includes the client name, the laboratory identification numbers assigned to each bulk sample upon receipt by the laboratory sample custodian, and the field number assigned to each bulk sample during the building inspection. The composition of the bulk sample is reported in percentages of asbestos (i.e., chrysotile, amosite, crocidolite, or other) and non-asbestos (i.e., cellulose, fiberglass, synthetic, or other) components. The original laboratory reports are presented in Figure 3-1 in Section 3.2.
There is another factor that relates to sample analysis. Assume that seven bulk samples were taken from a large homogeneous area. If the first sample analyzed by the laboratory is positive for asbestos fibers, there is no need to analyze the other six samples; per AHERA protocol, one positive sample is enough to determine that the entire area contains ACBM. If the first sample is negative, then further analysis is required until either a positive sample is found or until all seven samples are tested as negative.
3.2 RESULTS OF BULK SAMPLE ANALYSES
The results of the analyses of all samples that were suspected to contain asbestos are summarized in Table 3-1. This table lists:
The location from where the sample was collected.
A description of the material sampled.
The identification number assigned to the sample by the evaluation specialist in the field.
The identification number assigned to the sample by the analytical laboratory.
If asbestos was found in the sample, the percentage of asbestos present and the type of asbestos.
The homogeneous area from which the sample was collected.
Indication as to whether or not the sample was friable at the time of collection (as required by the EPA's National Emission Standard for Hazardous Air Pollutants ).
The actual laboratory reports are presented in Figure 3-1. These reports also list the nature of the non-asbestos material that was identified in each sample.
