About this file
This is Amendment #7 to a Request for Applications (RFA) issued by the Oregon Health Authority (OHA) in partnership with the Oregon Department of Human Services (ODHS) for Clinical Transformation Consultant (CTC) Services supporting the Patient-Centered Primary Care Home (PCPCH) Program. The RFA seeks to establish up to five contracts to provide clinical consultation services to primary care clinics, including conducting site visits, collaborating with OHA staff to verify PCPH measures, writing site visit reports, and providing post-visit consultation services. The original RFA closing date has been extended from April 30, 2023, to March 31, 2024, with applications due by 2:59:00 PM Pacific Time on that date through the OregonBuys system. The initial contract term is anticipated to run from January 6, 2023, through December 31, 2023, for an initial period not to exceed 12 months per contract.
The cumulative initial contract value for all positions is $75,000.00, with individual contracts not to exceed $15,000 each. Contracts include options to extend for additional time and money up to a maximum of five years total, with a not-to-exceed amount of $150,000 per individual contract. Amendment #7 also reflects administrative changes, including updates to the issuing office address to 500 Summer Street NE, E-03, Salem, Oregon 97301, and the sole point of contact email address to suzanne.christman@odhsoha.oregon.gov. Contractors should understand that contract awards provide no guarantee of minimum work, with services to be provided on an as-needed basis in accordance with a fully executed contract.
