About this file

This file is a Proposer Information and Certification Form (Exhibit 2) for Oregon Department of Human Services (ODHS) Request for Quotation (RFQ) #5523, Opportunity Notice #S-10000-00004811, related to the Anderson Readiness Center Parking Area Improvements Project in Marion County, Salem, Oregon. The form requires proposers to provide organizational information including legal name, address, state of incorporation, entity type, contact details, and Oregon Business Registry Number. Proposers must certify their understanding and acceptance of RFQ requirements, acknowledge receipt of all amendments, and confirm that their quote constitutes a firm offer valid for 60 calendar days following the Bid Opening Date. The contract, if awarded, requires performance of the negotiated scope of work and adherence to established performance standards for an initial two-year term.

The certification form mandates that proposers comply with Oregon state tax laws, employment discrimination statutes, wage equity requirements under ORS 652.220, and sexual harassment prevention policies for contracts anticipated to exceed $150,000. Proposers must certify they are not listed on federal exclusion databases (OFAC Specially Designated Nationals list and System for Award Management), possess no conflicts of interest with the State of Oregon, and have prepared their responses independently without collusion. The form requires attestation that all quote contents are truthful and accurate, with acknowledgment that false statements or material omissions may constitute false claims under the Oregon False Claims Act (ORS 180.750-180.785). Additionally, proposers certify compliance with broadband service provisions under Oregon Laws 2018 Chapter 88 and confirm non-discrimination in employment practices, employee wage discussions, and subcontract awards to disadvantaged business enterprises, minority-owned businesses, woman-owned businesses, service-disabled veteran-owned businesses, and emerging small businesses.
