About this file
These are construction drawings for a flooring replacement project at the Department of Veterans Affairs Medical Center in Bath, New York, specifically for Room 140, Building 76. The project involves replacing existing flooring with new LVT (luxury vinyl tile) flooring with 6-inch flashcove base in a radiography/medical imaging facility. The drawings include architectural, structural, electrical, and support layout plans prepared by Philips as equipment manufacturer drawings, with construction oversight by the Office of Construction and Facilities Management. Key project details include a reported ceiling height of 9 feet (2,743 mm), recommended minimum ceiling height of 8 feet 9 inches (2,667 mm), and recommended maximum of 10 feet 6 inches (3,200 mm). Architectural requirements specify demolition of existing flooring and cove base, wall furring-out, patch and paint work, and installation of new LVT flooring. The project includes ICRA (Infection Control Risk Assessment) dust control measures with negative air pressure requirements, tacky mats, and designated construction zones to protect occupied adjacent areas.
Electrical requirements specify a 112.5 kVA minimum power supply with three-phase, three-wire power at 400, 440, 460, or 480 VAC, 60 Hz, with a 3-pole 50-amp circuit breaker (slow blow) at the main disconnect. The project includes comprehensive conduit and raceway systems for power, signal, data, high-tension, and video cable separation, with detailed specifications for wall boxes, floor boxes, terminal blocks, and cable outlet locations. Structural support includes Unistrut ceiling support with P1001 channels spaced at 4-foot 3-inch intervals, floor and wall anchors for the Diagnost TH table base (requiring four M12 anchors), wall-mounted BuckyDiagnost VS digital equipment with multiple anchor options, and a Wi-Fi access point. All dimensions are referenced to finished wall surfaces, and the contractor is responsible for verifying existing conditions, providing all necessary conduit, boxes, and installation labor, with Philips supplying pre-terminated cables and equipment installation oversight.

PRIOR TO INITIATING WORK, CONTRACTOR WILL ASSESS THE SCOPE OF THE CONSTRUCTION ACTIVITIES AND DETERMINE THE POTENTIAL RISK GROUPS INVOLVED ON THE LOCATION OF ACTIVITIES AND EXTENT AND DURATION OF THE WORK. LOW RISK GROUPS ARE CONSIDERED "OFFICE WORKERS", MEDIUM RISK GROUPS ARE CONSIDERED ALL INPATIENT AND OUTPATIENT AREAS SUCH AS PEDIATRIC & ADULT OPERATING/DELIVERY ROOMS, CATH LABS, MYELO SUPPRESSIONS UNITS, ICU'S, DIALYSIS, NURSERIES AS WELL AS CLINIC AREAS (CLINICS ASSOCIATED WITH HEMATOLOGY, ONCOLOGY, PEDIATRIC INFECTION DISEASE, OR TRANSPLANT SERVICES) AND SERVICE AREAS (CENTRAL PROCESSING, STERILE PROCESSING, FOOD PREP & SERVICE AREA AND PHARMACIES. THE VAMC INFECTION CONTROL GROUP WILL CONFIRM AND APPROVE THE DESIGNATION PROVIDED IN THIS SUBMITTED DUST CONTROL PLAN. VAMC IS ALSO RESPONSIBLE FOR ANY AND ALL INFECTION CONTROL MEASURES DETERMINED & ESTABLISHED BY VAMC POLICY, SUCH AS THE RELOCATIONS OF HIGH RISK PATIENTS, APPROVING ALTERNATE TRAFFIC PATHS FOR CONSTRUCTION WORKERS OR ESTABLISH ALTERNATE TRAFFIC PATTERNS FOR STAFF, PATIENTS & VISITORS. THE PLAN SHOULD BE REVISED ACCORDING TO SPECIFIC VAMC POLICIES OR CHANGING CONDITIONS AT THE SITE AS THEY MAY OCCUR.
BATH RENOVATION
FOR THE PURPOSE OF THIS RENOVATION, THE CONSTRUCTION WORK IS CONSIDERED MEDIUM RISK AND COMPRISES WORK WHICH COULD GENERATE MODERATE TO HIGH LEVELS OF DUST NOT WITHIN THE CONFINES OF A SUITE OR OFFICE AND INCLUDES EXTENSIVE CONSTRUCTION WORK (DEMOLITION) THROUGHOUT THE FIRST FLOOR. THE DUST CONTROL PLAN MUST BE DISTRIBUTED AND REVIEWED BY ALL CONTRACTORS PERFORMING WORK ON THIS CONSTRUCTION PROJECT.
A. EXTERNAL DEMOLITION & CONSTRUCTION ACTIVITIES
- DETERMINE IF THE IMPACTED AREAS OF THE FACILITY CAN OPERATE TEMPORARILY ON RE-CIRCULATED AIR; IF FEASIBLE, SEAL
OF ADJACENT AIR INTAKES.
- IF THIS IS NOT POSSIBLE OR PRACTICAL, CHECK THE LOW-EFFICIENCY FILTER BANKS FREQUENTLY & REPLACE AS NEEDED TO
AVOID BUILDUP OF PARTICULATE.
- SEAL WINDOWS & REDUCE WHEREVER POSSIBLE OTHER SOURCES OF OUTSIDE AIR INTRUSION (OPEN DOORS IN CORRIDORS)
ESPECIALLY IN PROTECTIVE ENVIRONMENT (PE) AREAS.
B. INTERNAL DEMOLITION & CONSTRUCTION ACTIVITIES DUST & DEBRIS CONTROL
- BARRIER SYSTEMS: THE AREA SHOULD BE ISOLATED, AS THE PROJECT REQUIRES. SMALL, SHORT DURATION PROJECTS GENERATING MINIMAL DUST MAY SE FIRE-RATED PLASTIC SHEETING, BUT SHOULD BE SEALED AT FULL CEILING HEIGHT WITH AT LEAST 2-FOOT OVERLAPPING FLAPS FOR ACCESS TO ENTRY. PROJECTS THAT PRODUCE MODERATE TO HIGH LEVELS OF DUST REQUIRE RIGID, DUST-PROOF, & FIRE-RATED BARRIERS WALLS (E.G DRYWALL) WITH CAULKED SEAMS FOR A TIGHT SEAL EXTENDING FLOOR TO CEILING. SEAL OFF AND BLOCK AIR VENTS IF RIGID BARRIERS ARE USED FOR CONTAINMENT. LARGE DUST PROJECTS MAY NEED AN ENTRY VESTIBULE FOR CLOTHING CHANGES AND TOOL STORAGE AND TIGHT SEALS SHOULD BE MAINTAINED AT THE FULL PERIMETER OF WALLS & WALL PENETRATIONS. AN INTERIM PLASTIC DUST BARRIER (MINIMUM 4-MIL) MAY BE REQUIRED TO PROTECT THE AREA WHILE THE RIGID IMPERVIOUS BARRIER IS BEING CONSTRUCTED. ANY DUST MUST BE IMMEDIATELY CLEANED IF TRACKED OUTSIDE OF THE CONSTRUCTION BARRIER. UPON COMPLETION OF THE CONSTRUCTION PROJECT DUST BARRIER MUST BE REMOVED CAREFULLY TO MINIMIZE SPREADING OF DUST & THE CONTRACTOR PERSONNEL MUST MONITOR & PERFORM BARRIER MAINTENANCE & BE EDUCATED TO NOTICE SIMPLE CLUES SUCH AS ACCUMULATIONS OF VISIBLE DUST EVIDENCED BY FOOTPRINTS, OPENED DOORS/WINDOWS EVIDENCED BY PRESENCE OF INSECTS & FLIES, WET
CEILING TILES, ETC.
- TRAFFIC CONTROL; DESIGNATED ENTRY & EXIT PROCEDURES MUST BE DEFINED. EGRESS PATHS SHOULD BE FREE OF DEBRIS;
DESIGNATED ELEVATORS SHOULD BE USED DURING SCHEDULED TIMES; AND ONLY AUTHORIZED PERSONNEL SHOULD BE ALLOWED TO ENTER THE CONSTRUCTION ZONE. SIGNAGE SHOULD DIRECT PEDESTRIAN TRAFFIC AWAY FROM TH E
CONSTRUCTION AREA & MATERIALS.
-DEMOLITION DEBRIS: DEBRIS SHOULD BE REMOVED IN CARTS WITH TIGHTLY FITTED COVERS, USING DESIGNATED TRAFFIC ROUTES. EFFORTS SHOULD BE REMOVED DAILY & AT TIMES SPECIFIED BY THE VAMC. IF CHUTES ARE USED TO DIRECT DEBRIS OUTSIDE, HIGH EFFICIENCY PARTICULATE AIR (HEPA) FILTERED NEGATIVE AIR MACHINES SHOULD BE USED, & THE CHUTE OPENING SHOULD BE SEALED WHEN NOT IN USE. FILTERS SHOULD BE BAGGED AND SEALED BEFORE BEING TRANSPORTED OUT OF THE CONSTRUCTION AREA. THE CONTRACTOR MUST NOT HAUL DEBRIS THROUGH PATIENT-CARE AREAS WITHOUT PRIOR
APPROVAL FROM THE VAMC.
- EXTERIOR WINDOWS: WINDOWS SHOULD BE SEALED TO MINIMIZE INFILTRATION FROM ANY ADJACENT EXCAVATION DEBRIS.
- CEILINGS THAT ARE REMOVED/MODIFIED TO ALLOW CONSTRUCTION OF ICRA BARRIERS SHOULD BE REINSTALLED TO
PRE-CONSTRUCTION CONDITIONS.
DUST CONTROL PLAN NOTES/DUST CONTROL PLAN HAZARD ASSESSMENT
- TRAFFIC CONTROL; DESIGNATED ENTRY & EXIT PROCEDURES MUST BE DEFINED. EGRESS
PATHS SHOULD BE FREE OF DEBRIS; DESIGNATED ELEVATORS SHOULD BE USED DURING
SCHEDULED TIMES; AND ONLY AUTHORIZED PERSONNEL SHOULD BE ALLOWED TO ENTER THE
CONSTRUCTION ZONE.
EFFORTS SHOULD BE REMOVED DAILY & AT TIMES
SPECIFIED BY THE VAMC. IF CHUTES ARE USED TO DIRECT DEBRIS OUTSIDE, HIGH EFFICIENCY PARTICULATE AIR (HEPA) FILTERED NEGATIVE AIR MACHINES SHOULD BE USED, & THE CHUTE

HAUL DEBRIS THROUGH PATIENT-CARE AREAS WITHOUT PRIOR APPROVAL FROM THE VAMC.
- EXTERIOR WINDOWS: WINDOWS SHOULD BE SEALED TO MINIMIZE INFILTRATION FROM ANY
ADJACENT EXCAVATION DEBRIS.
- CEILINGS THAT ARE REMOVED/MODIFIED TO ALLOW CONSTRUCTION OF ICRA BARRIERS
SHOULD BE REINSTALLED TO PRE-CONSTRUCTION CONDITIONS.
VENTILATION & ENVIRONMENTAL CONTROLS
- AIR SYSTEM FLOW: DETERMINE WHTHER THE CONSTRUTION AREA USES FRESH/OUTSIDE OR
RE-CIRCULATED AIR, FILTERS SHOULD BE ADDED OR RETURN VENTS COVERED AS NEEDED WITH
FILTER MATERIAL OR PLASTIC. AIR MUST FLOW FROM CLEAN TO DIRTY AREAS.
- NEGATIVE AIR PRESSURE: THE AIR WITHIN THE CONSTRUCTION AREA MUST BE NEGATIVE WITH RESPECT TO SURROUNDING AREAS & WITH NO DISRUPTION OF AIR SYSTEMS OF ADJACENT AREAS.
USE OF THE NEGATIVE AIR PRESSURE SYSTEM WITHIN THE ENCLOSURE TO REMOVE DUST SHOULD
PASS AIR THROUGH AN INDUSTRIAL GRADE, PORTABLE HEPA FILTER CAPABLE OF FILTRATION
RATES OF 300-800 CUBIC FEET PER MINUTE (FT3/MIN), OR EXHAUST AIR DIRECTLY TO THE OUTSIDE
IF APPROVED BY VAMC. IF EXHAUST MUST BE TIED INTO A RE-CIRCULATED AIR SYSTEM, A
PRE-FILTER & HEPA FILTER SHOULD BE USED BEFORE EXHAUST TO PREVENT CONTAMINATION OF
THE DUCTS.
- ADJACENT AREAS: THE STATUS OF SEALED PENETRATIONS & INTACT CEILING SHOULD BE
VERIFIED DAILY.
- AIR EXCHANGE RATES & PRESSURE RELATIONSHIPS: VAMC AND/OR CONTRACTOR WILL VERIFY & MAINTAIN PROPER RATES IN CRITICAL AREAS NEAR CONSTRUCTION ACTIVITY & ENSURE AIR IS NOT BEING RE-CIRCULATED WITHOUT FILTRATION FROM THE CONSTRUCTION AREA ELSEWHERE. VAMC
WILL MAKE DETERMINATION OF PROVIDING FOR THE ACCOUNTABILITY AND FREQUENCY OF
TESTING AIR PRESSURE THROUGHOUT THE PROJECT.
- MANOMETERS SHOULD BE LOCATED ON ICRA PARTITION DOOR & ON THE NON-CONSTRUCTION
SIDE OF THE PARTITION.
CONTAMINATION OF PATIENT ROOMS
- WORK SITE CLOTHING: CONTRACTOR PERSONNEL CLOTHING SHOULD BE FREE OF LOOSE SOIL & DEBRIS BEFORE LEAVING THE CONSTRUCTION AREA. IF PROTECTIVE APPAREL IS NOT WORK (E.G
COVERALLS, FOOTGEAR & HEADGEAR) A HEPA-FILTERED VACUUM SHOULD BE USED TO REMOVE
DUST FROM CLOTHING BEFORE LEAVING THE BARRICADE. IF PROTECTIVE APPAREL IS UTILIZED
THE CONTRACTOR MUST CONSTRUCT A SPACE OR ANTEROOM FOR CHANGING CLOTHING &
STORING EQUIPMENT (DESIGNATED AREA). ALL EQUIPMENT, TOOLS, TOOL CARTS, MATERIALS, ETC.
TRANSPORTED THROUGH OCCUPIED AREAS MUST BE MADE FREE FROM DUST & MOISTURE BY
VACUUMING & WET WIPING BEFORE THEIR REMOVAL FROM THE CONSTRUCTION ZONE OR WORK
AREA.
- CONTRACTOR CLEANING: THE CONSTRUCTION ZONE SHOULD BE MAINTAINED IN A CLEAN
MANNER BY CONTRACTORS & SWEPT OR HEPA-VACUUMED DAILY OR MORE FREQUENTLY AS
NEEDED TO MINIMIZE DUST. ADJACENT AREAS THAT MAY BE IMPACTED BY THE CONSTRUTION
SHOULD BE DAMP MOPPED DAILY OR MORE FREQUENTLY AS NEEDED. WALK-OFF MATS WITH
TACKY OR ADHESIVE SURFACES SHOULD BE UTILIZED TO MINIMIZE TRACKING OF HEAVY DIRT &
DUST FROM CONSTRUCTION AREAS.
NOTES:
1. ENSURE RENOVATED AREA IS COMPLETELY SEALED OFF FROM SURROUNDING AREAS.
2. MAINTAIN MEANS OF EGRESS DURING CONSTRUCTION. RESTRICT ACCESS TO WORK AREA DURING
CONSTRUCTION.
3. PROVIDE AND MAINTAIN CLEAN TACKY MATS AT ENTRANCE / EXIT OF ICRA BARRIER TO MINIMIZE
TRACKING CONSTRUCTION DEBRIS THROUGH THE BUILDING.
4. COORDINATE PHASING SCHEDULE WITH COR PRIOR TO STARTING CONSTRUCTION.
5. CEILINGS THAT ARE REMOVED/MODIFIED TO ALLOW CONSTRUCTION OF ICRA BARRIERS ARE THE
CONTRACTOR'S RESPONSIBILITY TO BE REINSTALLED TO PRE CONSTRUCTION CONDITIONS.

· Philips does not specify the overhead equipment support structure. Unistrut (or equal) may or may not be used. If Unistrut are used, it is up to Unistrut and the structural engineer for the project to determine which of it's products are appropriate for each project.
· Finished ceiling must NOT be lower than the bottom of the Unistrut in order to prevent damage to the finished ceiling during the installation of clip rails.
· Finished ceiling height to be 1/4" above bottom of Unistrut.
· Nothing shall be attached to the Unistrut with any fastener that protrudes into the Unistrut which would interfere with positioning of the fixing block.
· Fixing blocks for Philips ceiling rails (clip rails) are designed to be installed in P1001 Unistrut.
· The inside of the Unistrut must be clear of obstructions (including paint).
· Unistrut elements must be rigid and comply with the ceiling structure requirements. See SN sheet, item #4 "Ceiling Support

The use of 90 degree ells is not acceptable. On ceiling duct and wall duct use 45 degree bends at all corners. All intersecting points in duct to have cross over tunnels supplied and installed by contractor to maintain separation of cables.
4. Raceway / Conduit Raceway / Conduit point - to - point runs shall be as direct as possible. Empty conduit runs used for cables may require pull boxes located along the run. Consult with Philips. A Greenlee measuring tape (part no. 435) shall be installed in each conduit run. Best practice to name the physical conduit. All conduits which enter duct prior to their termination point must maintain separation from other cables via use of dividers, cross over tunnels, or conduit supplied and installed by contractor from entrance into duct to exit from duct.
5. Conductors All conductors, separately specified, shall be 75°C stranded copper, rung out and marked.
6. Disconnecting Means A disconnecting means shall be provided as separately specified.
7. Warning Lights and Door Switches "X-ray on" warning lights and x-ray termination door switches should be provided at all entrances to x-ray rooms as required by code.
8. Dimmer Switches X-ray room lights should be provided with dimmer switches.

Electrical Notes
1. The contractor will supply & install all breakers, shunt trip and incoming power to the breakers. The exact location of the breakers and shunt trips will be determined by the architect or contractor.
2. The contractor shall supply & install all pull boxes, raceways, conduit runs, stainless steel covers, etc. Conduit/raceways must be free from burrs and sharp edges over its entire length. A Greenlee pull string/measuring tape (part no. 435, or equivalent) shall be provided with conduit runs.
3. All pre-terminated, cut-to-length cables will be supplied and installed by Philips. See Conduit List for information regarding all other cables (e.g. cables to breakers, etc.).
4. Provide and install 4 - 2"(50mm) diameter chase nipples between adjacent wall boxes (not required if raceway installed above and below wall boxes).
5. Electrical raceway shall be installed with removable covers. The raceway should be accessible for the entire length. In case of non - accessible floors, walls and ceilings, an adequate number of access hatches should be supplied to enable installation of cabling.
Approved conduits may be substituted. All raceways will be designed in a manner that will not allow cables to fall out of the raceway when the covers are removed. In most cases, this will require above-ceiling raceway to be installed with the covers removable from the top. Raceway system as illustrated on this drawing are based upon length of furnished cables. Any changes in routing of raceway system could exceed maximum allowable length of furnished cables. Conduit or raceway above-ceiling must be kept as near to finished ceiling as possible.
6. Conduit sizes shall be verified by the architect, electrical engineer or contractor, in accordance with local or National Electrical Codes, whichever governs.
7. Convenience outlets are not illustrated. Their number and location are to be specified by the customer/architect.
8. All sections of raceway and conduit shall be grounded with an independent #6AWG green wire that is to be attached using solderless lugs All ceiling mounted structural support members and ceiling plates shall also be grounded. All grounding connections, terminals, etc. shall be installed in a manner to provide accessibility for inspection, maintenance, repair, etc.
9. The contractor is to ensure system cables that run from the system cabinet to the table are not run under the slab in ground floor installations. They need to be run in a trough or a raceway in the floor per NEC section 390 Underfloor Raceway. Conduits may not be allowed in a basement slab. The cables from the cabinets to the table are not certified in a wet environment/area.
10. Applied cable connectors cannot be removed for installation. Site must select a suitable size for cable ducts and wall outlets so connectors and cables can pass through.
11. Flex conduit does not meet Philips installation requirement and is NOT ALLOWED for substitution for EMT/Rigid conduit materials. Any deviation for conduit material requires written permission from the Philips project manager.

Power Quality Guidelines
1. Power supplied to medical imaging equipment must be separate from power feeds to air conditioning, elevators, outdoor lighting, and other frequently switched or motorized loads. Such loads can cause waveform distortion and voltage fluctuations that can hinder high quality imaging.
2. Equipment that utilizes the facility power system to transmit control signals (especially clock systems) may interfere with medical imaging equipment, thus requiring special filtering.
3. The following devices provide a high impedance, nonlinear voltage source, which may affect image quality: static UPS systems, series filters, power conditioners, voltage regulators.
Do not install such devices at the mains supply to medical imaging equipment without consulting Philips installation or service personnel.
4. Line impedance is the combined resistance and inductance of the electrical system and includes the impedance of the power source, the facility distribution system, and all phase conductors between the source and the imaging equipment. The minimum conductor size is based on the total line impedance and NEC requirements. Impedance calculations are to be performed by an electrical engineer).
