About this file
This document provides instructions to offerors for solicitation number FA8224-20-R-0010 seeking preventative maintenance, repair, relocation and disposal services for carousels, vertical lift modules and Lektrievers. Offerors must submit three volumes - a price proposal, technical evaluation plan, and past performance information.

The technical proposal must demonstrate experience maintaining the specified equipment, responding to service requests within timeframes, and acquiring original equipment manufacturer parts including obsolete items. The past performance submission should include up to three recent and relevant contracts demonstrating compliance, with customer references and ratings. Electronic proposals are due by the specified date. The Air Force Materiel Command aims to award a contract for these services at Hill Air Force Base in Utah.

52.212-1 Addendum Instructions to Offerors (ITO) Lektriever Carousel/Vertical Lift Modules Maintenance
52.212-1 ADDENDUM - OFFER PREPARATION INSTRUCTIONS
A. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements, in addition to those identified as evaluation factors or sub factors. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The Offer shall consist of three (3) volumes:
1. Volume I - Price Proposal
2. Volume II - Technical Evaluation and Quality Control Plan
3. Volume III - Past Performance
B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer’s opinion, adequate price competition exists no additional cost information will be requested. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists Offerors may be required to submit information other than cost or pricing data to support a determination of price reasonableness.
C. SPECIFIC INSTRUCTIONS:
1. VOLUME I - PRICE - Submit one (1) electronic copy.
(a.) Complete blocks 12, 17a, 30a, 30b, and 30c for the RFP Section A of the SF1449. In doing so, the offeror accedes to the contract terms and conditions as written in the SOLICITATION, with attachments. The SOLICITATION constitutes the model contract.
(b.) Offerors shall input into the TEP Worksheet, located in Section J Attachment - TEP Worksheet. The Total Evaluated Price (TEP) will be the sum of the proposed prices for all CLINs including the six month option. The TEP will be used for evaluation purposes only and does not constitute the total contract price, however, all prices that comprise the TEP are contractually binding and thus all prices must be populated.
(c.) Submit a copy of the completed TEP worksheet in the Price Volume (Volume I) in Microsoft Excel (non PDF format). All cells of the TEP worksheet must be filled in. An omitted CLIN price/cell may result in an incomplete price submission.
(d.) Cost or Pricing Information Requirements - IAW FAR 15.402, 15.403-1(b) and 15.403-3(a), “information other than cost or pricing data” may be required to support price reasonableness. If, after receipt of proposals, the CO determines that there is insufficient information available to determine the reasonableness of the proposed pricing the offeror shall be required to submit additional information other than cost or pricing data. If, during the source selection process, the CO determines that adequate competition no longer exists, then cost and pricing data may be required.
(e.) Rounding - All dollar amounts provided shall not extend beyond two decimal places.
(f.) The Data CLIN is to be left blank for all years. The Data CLINs are Not Separately Priced line items.
2. VOLUME II - TECHNICAL EVALUATION – Limited to no more than twelve (12) pages. Submit one (1) electronic copy. Please describe your capability and experience relating to each section of the PWS. The proposal shall not simply restate or rephrase the Government’s requirement, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. An Offeror shall assume that the Government has no prior knowledge of their facilities and experience, and will base its evaluation on the information presented in the proposal. The following technical sub factors will be evaluated in accordance with FAR 52.212-2 Addendum of the solicitation.
a. Sub-factor 1: Certificates for Kardex/Remstar and Sencorp White- The offeror shall submit certificates from the OEM for understanding of the OEM’s machines identified in Appendix B of the PWS. The OEMs certificates of training are required for Remstar and SencorpWhite. (IAW PWS 2.5)
b. Sub-factor 2: Plan for response times after notification of inoperative equipment:
The offeror's must demonstrate a sound approach how the response time will be achieved (IAW PWS 2.1.5). Incomplete descriptions and copies of the Performance Work Statement (PWS) are unacceptable.
c. Sub-factor 3: Capability to acquire OEM parts:
The offeror shall provide a detailed part requisition plan that outlines their approach and demonstrates their ability to acquire OEM parts, including those parts that are obsolete or hard to find that would not invalidate a warranty or result in the OEM refusing to support the equipment. (IAW PWS 2.3)
D. VOLUME III - PAST PERFOMANCE – Limited to no more than two (2) pages per contract listed. Only references for relevant past efforts/contracts are desired. Submit one (1) copy with proposal.
1. Offerors shall, if available, submit recent and relevant contracts (maximum of three contracts) as defined below. Performance as subcontractor may be included; the offeror shall include contract number and prime contractor. The Government will consider this information, along with information obtained from other sources (e.g., Past Performance Information Retrieval System (PPIRS), Contractor Performance Assessment Reports System (CPARS), customer interviews, etc.). Offeror may use past performance of subcontractor to satisfy recent and relevant past performance. If this is a first time joint venture, each party to the joint venture must provide a list of past relevant contracts (maximum of three). Furnish the following information for each contract listed:
| (i) | Company/Division name | |

The Government will not consider performance on contracts where performance was concluded more than three years prior to the solicitation issue date or contracts awarded. The contract/task order in its entirety may be evaluated for past performance if any part of the contract performance falls within the three year timeframe.
3. Relevancy – is defined as work that involves approximately the same type(s) of services described in the RFP, PWS, 52.212-1 Addendum, and 52.212-2 Addendum. The offeror shall address any performance problems/issues as instructed in 52.212-1 Addendum and explain the corrective action taken or current status.
4. Offerors may address any past performance problems or issues that the Government may identify and explain corrective action taken and current status. The Government will consider this information in determining the offeror’s past performance rating. An offeror’s failure to address performance problems or issues within this volume does not preclude the Government from using such information in determining a past performance rating.
5. If a joint venture is contemplated, provide complete information as to the joint venture. Include any relevant and recent past performance information on previous joint ventures with the same partner. If this is a first time joint venture, each party to the joint venture should provide a list of past relevant contracts (maximum of three).
6. Quality and Satisfaction Rating for Contracts Completed in the Past Three Years: Provide any information currently available (letters, metrics, customer surveys, independent surveys, etc.) which demonstrates customer satisfaction with overall job performance and quality of completed product for relevant past efforts/contracts (similar in scope and magnitude of effort and complexities this solicitation requires). In addition, explain corrective actions taken in the past, if any, for substandard performance and any current performance problems such as cost overruns, extended performance periods, numerous warranty calls, etc.
