About this file

The document is a Certificate of Interested Parties (Form 1295) instructions and form from Fort Bend Independent School District (FBISD), related to House Bill 1295 and Texas Government Code Section 2252.908. The document requires vendors to electronically complete, print, sign, and submit Form 1295 with their proposals, disclosing any interested parties who have a controlling interest in the business entity or who actively participate in facilitating or negotiating the contract terms. The form must be filed with the Texas Ethics Commission using their online filing application, even if no interested parties exist.

The specific form provided shows Peter James from Missouri City, TX as an interested party with a controlling interest, with the form executed on June 5, 2022. The form is part of FBISD's compliance requirements, prohibiting the district from entering into contracts with business entities that do not submit this disclosure. The document emphasizes that "business entity" includes various business structures like sole proprietorships, partnerships, and corporations, and all vendors must complete this form as part of the proposal submission process.
